**Contract for Social Services and Other Specific Services**, to which the provisions of the Act of 11 September 2019 - Public Procurement Law (Journal of Laws of 2026, item 793), hereinafter referred to as the Pzp Act, applicable to classic contracts, with a value equal to or exceeding the EU thresholds – if the value of the contract expressed in PLN is equal to or exceeds the equivalent of EUR 750,000, taking into account the changes resulting from Section IV, Section 4 of the Pzp Act. The procedure for awarding the public contract is conducted in the form of an unrestricted tender based on Article 132-139 in conjunction with Article 129(2) of the Act of 11 September 2019, Public Procurement Law (Journal of Laws of 2026, item 793). The subject of the contract is the implementation of animation and educational-cultural actions integrating the residents of the area of Project 9 ROCŁ and its immediate vicinity, enhancing their social potential, strengthening their interest and encouraging the use of public spaces in the area in the form of events, workshops and actions in urban space. The contract aims to activate residents, strengthen social bonds and increase interest in using public urban spaces. The contract consists of 17 tasks in accordance with the Subject of the Contract Description. A detailed description of the subject of the contract is in Annex No. 1 to SWZ and in the contract template in Annex No. 7 to SWZ. (Numbering in accordance with SWZ): 40.1 The Contracting Authority does not envisage: 40.1.1. concluding a framework agreement; 40.1.2. submitting variant and partial offers; 40.1.3. settlement in foreign currencies; 40.1.4. electronic auction; 40.1.5. reimbursement of participation costs; 40.1.6. requirement or possibility of submitting offers in the form of electronic catalogues or attaching electronic catalogues to the offer; 40.1.7. the Contractor carrying out a local visit or the Contractor checking the documents necessary for the implementation of the contract available at the Contracting Authority's premises; 40.1.8. awarding the contract under the right of option based on Article 441 of the Pzp Act. 40.2 Scope and conditions for changing the concluded contract - matters related to the contract are regulated in the contract template Annex No. 7 to SWZ. 40.3 The Contracting Authority did not conduct preliminary market consultations before initiating the procedure. The tender shall consist of the following documents: 15.1 Offer form – prepared according to the template - Annex No. 2 to SWZ (signed with a qualified electronic signature). 15.2 Price form – prepared according to the template - Annex No. 2a to SWZ (signed with a qualified electronic signature). 15.3 Contractor's declaration regarding the grounds for exclusion under Article 5k of Regulation 833/2014 concerning restrictive measures in connection with Russia's destabilising actions in Ukraine and Article 7(1) of the Act on special solutions in the field of counteracting the support of aggression on Ukraine and serving to protect national security, submitted pursuant to Article 125(1) of the Pzp Act, in accordance with the template constituting Annex No. 5a to SWZ (signed with a qualified electronic signature). In the case of contractors jointly applying for the contract, this declaration is submitted by each of these Contractors (this also applies to partners of a civil law partnership). 15.4 Declaration of the entity providing resources regarding the grounds for exclusion under Article 5k of Regulation 833/2014 concerning restrictive measures in connection with Russia's destabilising actions in Ukraine and Article 7(1) of the Act on special solutions in the field of counteracting the support of aggression on Ukraine and serving to protect national security, submitted pursuant to Article 125(5) of the Pzp Act, in accordance with the template constituting Annex No. 5b to SWZ (if applicable) (signed with a qualified electronic signature). 15.5 Power of attorney/powers of attorney for the person/persons signing the offer, if the offer is signed by a proxy, (if applicable). 15.6 In the case of tenders submitted by Contractors jointly applying for the award of the contract - power of attorney to represent all Contractors jointly applying for the award of the contract, or an agreement on cooperation, from which the relevant power of attorney will result. 15.7 Obligation of the entity providing its resources for the needs of the Contractor submitting the offer, (if applicable - obligation template Annex No. 8 to SWZ) or another evidential instrument confirming that the Contractor, when implementing the contract, will have the necessary resources of these entities (signed with a qualified electronic signature). 15.8 Declaration of division of obligations during the implementation of the contract, (if applicable) – declaration template Annex No. 9 to SWZ (signed with a qualified electronic signature). 15.9 Declarations and/or documents on the basis of which the Contracting Authority will assess the effectiveness of the reservation of information contained in the offer, constituting a business secret, within the meaning of the provisions on combating unfair competition, (if the Contractor reserves such information). 15.10 “Description of the idea and concept of implementing Task 3.1” - for the evaluation of the non-price criterion. The Contractor will attach to the offer the Description of the idea and concept of implementing Task 3.1 “Organisation and implementation of outdoor sports activities integrating the residents of the area of Project 9 ROCŁ and its immediate vicinity in 2027” (Task 3.1 in the Subject of the Contract Description (signed with a qualified electronic signature). 8.13 The entity providing its resources to the Contractor may, pursuant to Article 124(2) of the Public Procurement Law, instead of the appropriate evidential instruments, submit a certificate confirming the certification of public procurement contractors in the scope of non-exclusion or meeting the conditions of participation in the procedure.
