The subject of the contract is the successive delivery of specialist single-use equipment for the Hemodynamic-Coronarographic Laboratory (hereinafter referred to as: the goods or assortment) in the type and quantities specified in the Price Schedule, which is Annex 2 to the SWZ, together with the lease of the OCT system (hereinafter interchangeably referred to as: the equipment or the subject of the lease), described in detail in the List of Required Functions and Technical Parameters, which is Annex 6 to the SWZ, and in accordance with the design provisions of the contract, which is Annex 4 to this SWZ (the lease clause applies to task no. 1). 2. During the contract period, the Contractor will provide a free deposit of the goods referred to in paragraph 1 at the Contracting Authority's premises, hereinafter referred to in the contract as the "commission". The Contractor undertakes to deliver the goods to the Contracting Authority in the form of a replenishment of the above-mentioned commission and to transfer the ownership of the goods to the Contracting Authority at the time of their collection by the Contracting Authority from the commission, and the Contracting Authority undertakes to collect the goods and pay the agreed price for the goods collected from the commission. - applies to: task no. 1 item 1, 2; task no. 2 item 1, 2; task no. 6 item 1. 3. Detailed description of the subject of the contract according to tasks: Task no. 1: Delivery of equipment for PTCA; Task no. 2: Delivery of auxiliary equipment for PTCA; Task no. 3: Delivery of actively circulatory assisting pumps implanted percutaneousously; Task no. 4: Delivery of devices for invasive measurement of transstenotic pressure gradient; Task no. 5: Delivery of sets for closing persistent arterial ducts PDA; Task no. 6: Delivery of sets for embolisation of vessels and vascular malformations; in accordance with the detailed description of the subject of the contract contained in the Price Schedule, which is Annex 2 to the SWZ and Annex 6 to the SWZ (applies to task no. 1 and 3). The Contracting Authority informs that in the subject procedure conducted in the open tender procedure, it envisages the possibility of applying the procedure referred to in Article 139(1) of the p.z.p. Act. When applying the said procedure, the Contracting Authority will first carry out an examination and evaluation of the offers, and then carry out a subjective qualification of the Contractor, whose offer was the highest rated in terms of the absence of grounds for exclusion and the fulfilment of the conditions for participation in the procedure.
