1. The subject of the order is the execution and delivery of gift sets for children born in provincial healthcare entities in 2026 in the amount of 1 149 sets (basic order) with the option to extend (1 149 sets). 2. The Family Department plans to implement a task related to the symbolic gift for children born in provincial healthcare entities in 2026. These will be gift sets handed out in three provincial healthcare entities (Szpital Specjalistyczny im. Ludwika Rydygiera w Krakowie sp. z o.o., Szpital Wojewódzki im. Św. Łukasza w Tarnowie and Szpital Specjalistyczny im. Jędrzeja Śniadeckiego w Nowym Sączu) to the parents of children after birth. 3. The gift set will consist of a pillow, a blanket, a mascot, and a cotton bib, all branded with the WM logo, packed in a box with a window and tied with a ribbon. 4. The detailed description of the order subject is attachment no. 1D to the swz, which is also attachment no. 1 to the contract. 5. The conditions for the execution of the order are included in the draft provisions of the contract regarding the public order, which will be introduced into the content of this contract – contract template and constitute attachment no. 1C to the swz (hereinafter referred to as the contract template). 6. The Orderer envisages the possibility of using the option right within the framework of this order. The subject of the contract within the framework of the option right may be implemented up to 12 months from the date of completion of the basic order or until the allocated amount for the implementation of the order within the framework of the option right is exhausted. 7. The order within the framework of the option right will be carried out on the same terms as the basic order. 8. The Orderer may use the option right within 14 days before the end of the basic order implementation period by submitting a written or electronic statement to the Contractor and indicating the deadline for using the option right. 9. In the event that the Orderer does not use the option right, the Contractor shall not be entitled to any claims in this regard. 10. The estimated value of the basic order is 154 134,14 zł net. 11. The estimated value of the order covered by the option right is 154 134,15 zł net. 12. The requirement to carry out the order exclusively by contractors with the status of a protected workshop, social cooperative and other contractors, the main goal or main goal of whose organizationally separate units, which will implement the order, is the social and professional integration of socially marginalized persons. 13. If the order subject description contains: trade mark names, patents or origin, this should be treated solely as an aid in describing the order subject. In any case, equivalent products in terms of construction, parameters, purpose are acceptable. 14. CPV code: 39294100-0 Informational and promotional articles. 15. The amount allocated for the financing of the basic order is 200 000,00 zł gross.
