Contract for Social Services and Other Specific Services, to which the provisions of the Act of 11 September 2019 - Public Procurement Law (Dz.U. z 2026 r. poz. 793), hereinafter referred to as the Pzp Act, applicable to classical contracts, with a value equal to or exceeding the EU thresholds – if the value of the contract expressed in PLN is equal to or exceeds the equivalent of 750 000 euro, taking into account the changes resulting from Section IV, Chapter 4 of the Pzp Act. The procedure for awarding the public contract is conducted in the form of an unrestricted tender based on art. 132-139 in conjunction with art. 129(2) of the Act of 11 September 2019, Public Procurement Law (Dz. U. z 2026 r. poz. 793). The service is implemented as part of the Project, entitled “Revitalization of the Łódź City Centre (Project 9)”, co-financed under the Regional Programme European Funds for Łódź 2021-2027. The subject of the contract is: A service consisting of preparing and implementing educational and workshop activities counteracting digital exclusion, developing competencies in the field of new technologies, and supporting the social activation of residents and users of the revitalization area, with particular consideration for residents and users of the Project 9 ROCŁ area and its immediate vicinity. The actions implemented as part of the contract aim to develop the digital competencies and practical skills of the participants, as well as to increase the involvement of residents in using the offer of the Local Activity Centre at ul. Sienkiewicza 22 and to support the development and recognition of CAL as a place of education, integration, and local activity. This goal will be achieved through the organisation of 27 tasks (i.e. 10 tasks divided into editions) described in detail in the OPZ, which is an annex to the application, adapted to the needs of various groups of recipients, including people at risk of social and digital exclusion. The service is implemented as part of the Project, entitled “Revitalization of the Łódź City Centre (Project 9)”, co-financed under the Regional Programme European Funds for Łódź 2021-2027. A detailed description of the subject of the contract is provided in Annex No. 1 to the SWZ and in the contract template in Annex No. 7 to the SWZ. (Numbering consistent with the SWZ): 40.1 The Contracting Authority does not envisage: 40.1.1 concluding a framework agreement; 40.1.2 submitting variant and partial offers; 40.1.3 settlement in foreign currencies; 40.1.4 electronic auction; 40.1.5 reimbursement of participation costs; 40.1.6 requirement or possibility of submitting offers in the form of electronic catalogues or attaching electronic catalogues to the offer; 40.1.7 the Contractor carrying out a local inspection or the Contractor checking the documents necessary for the implementation of the contract available on-site at the Contracting Authority; 40.1.8 awarding the contract under the right of option based on art. 441 of the Pzp Act. 40.2 Scope and conditions for changes to the concluded contract - matters related to the contract are regulated in the contract template Annex No. 7 to the SWZ. 40.3 The Contracting Authority did not conduct preliminary market consultations before initiating the procedure. The offer consists of the following documents: 15.1 Offer Form – prepared according to the template - Annex No. 2 to the SWZ (signed with a qualified electronic signature). 15.2 Price Form – prepared according to the template - Annex No. 2a to the SWZ (signed with a qualified electronic signature). 15.3 Contractor's Declaration regarding the grounds for exclusion under art. 5k of Regulation 833/2014 concerning restrictive measures in connection with Russia's destabilising actions in Ukraine and art. 7(1) of the Act on specific solutions in the field of counteracting support for aggression on Ukraine and serving to protect national security, submitted pursuant to art. 125(1) of the Pzp Act, in accordance with the template constituting Annex No. 5a to the SWZ (signed with a qualified electronic signature). In the case of contractors jointly applying for the contract, this declaration is submitted by each of these Contractors (this also applies to partners of a civil law partnership). 15.4 Declaration of the entity providing resources regarding the grounds for exclusion under art. 5k of Regulation 833/2014 concerning restrictive measures in connection with Russia's destabilising actions in Ukraine and art. 7(1) of the Act on specific solutions in the field of counteracting support for aggression on Ukraine and serving to protect national security, submitted pursuant to art. 125(5) of the Pzp Act, in accordance with the template constituting Annex No. 5b to the SWZ (if applicable) (signed with a qualified electronic signature). 15.5 Power of attorney/powers of attorney for the person/persons signing the offer, if the offer is signed by a proxy (if applicable). 15.6 In the case of offers submitted by Contractors jointly applying for the contract - a power of attorney to represent all Contractors jointly applying for the contract, or a cooperation agreement, from which the relevant power of attorney will result. 15.7 Commitment of the entity providing its resources for the needs of the Contractor submitting the offer, (if applicable - commitment template Annex No. 8 to the SWZ) or another documentary evidence confirming that the Contractor, when implementing the contract, will have the necessary resources of these entities (signed with a qualified electronic signature). 15.8 Declaration of division of obligations during the implementation of the contract, (if applicable) – declaration template Annex No. 9 to the SWZ (signed with a qualified electronic signature). 15.9 Declarations and/or documents on the basis of which the Contracting Authority will assess the effectiveness of the reservation of information contained in the offer, constituting a business secret, within the meaning of the provisions on combating unfair competition, (if the Contractor reserves such information). 15.10 Description of the overall concept of implementing the Contract.
