1. The subject of the contract is the implementation of the task: Comprehensive investor supervision during the implementation of the contract “Adaptation of rooms in building A2 for the needs of the Specialist Outpatient Clinic Team in the Clinical and Teaching Centre of the Medical University of Lodz at ul. Pomorska 251 in the design and build formula” 2. The investment is carried out within the framework of the co-financing no. FENX.06.01-IP.03-0074/25-00/305/2026/57, Project no. FENX.06.01-IP.03-0074/25 entitled “Development and modernisation of the Specialist Outpatient Clinic Team CKD in the Central Clinical Hospital of the Medical University of Lodz” within the framework of the action FENX.06.01 Health Protection System, priority FENX.06 Health, Programme European Funds for Infrastructure, Climate, Environment 2021-2027. 3. A detailed description of the subject of the contract is contained in separate annexes – file DESCRIPTION OF CONTRACT (DOC) in part B of the contract notice. The place of execution of the contract is: Independent Public Health Care Institution Central Clinical Hospital of the Medical University of Lodz with its registered office in Lodz at ul. Pomorska 251, 92-213 Lodz, building A-2. 4. CPV designation according to the Common Procurement Vocabulary: 71247000-1 Construction work supervision; 71540000-5 - Other construction management services -- other CPVs in the contract description 5. In this tender procedure, the contracting authority has waived the division into lots as it considered that it was not possible to divide the contract into lots. The contracting authority has not divided the contract into lots due to the nature of the contract (further in the contract notice). 6. The contract will be implemented within a maximum period of 31.03.2028. 7. The scope and conditions of changes are contained in the model contract, which is an annex to the contract notice. 8.
