The subject of the contract is the successive delivery of consumable materials for continuous non-replacement therapies (hereinafter referred to as the goods), in the types and quantities specified in the price list constituting Annex 2 to the SWZ and in accordance with the planned provisions of the contract constituting Annex 4 to this SWZ, i.e. according to the tasks as follows: Task 1: Delivery of stentgrafts for the treatment of abdominal aortic aneurysms Task 2: Delivery of silver-impregnated plain peripheral prostheses Task 3: Delivery of silver-impregnated bifurcated plain prostheses Task 4: Delivery of silver-impregnated plain aortic prostheses Task 5: Delivery of high-pressure syringes for PTA Task 6: Delivery of embolectomy catheters with a lumen Task 7: Delivery of sets for stenting carotid arteries Task 8: Delivery of balloon catheters for stenting carotid arteries Task 9: Delivery of stents for the treatment of peripheral changes Task 10: Delivery of stents for renal vessels compatible with a 6F catheter Task 11: Delivery of sets for interventions below the knee. Task 12: Delivery of 0.014” and 0.018” guidewires Task 13: Delivery of devices for peripheral vessel embolization Task 14: Delivery of balloon catheters with a long system Task 15: Delivery of balloon-expandable stents for renal arteries Task 16: Delivery of diagnostic jackets Task 17: Delivery of leading catheters for embolization of aneurysms Task 18: Delivery of ultra-thin knitted vascular patches Task 19: Delivery of stents for renal vessels compatible with a 4F and 5F catheter Task 20: Delivery of hydrophilic guidewires and vascular closures Task 21: Delivery of universal balloon catheters. Task 22: Delivery of self-expanding stents for the popliteal artery in accordance with the detailed description of the subject of the contract contained in the Price List constituting Annex 2 to the SWZ. The contractor may submit an offer for one, several, or all parts. During the examination and evaluation of the offers, the Purchaser will consider each task separately. 2. For the duration of the contract, the Contractor will provide a free deposit of the goods referred to in paragraph 1 at the Purchaser's headquarters, hereinafter referred to in the contract as the "commission". Under the conditions specified in Annex 4 to the SWZ, the Contractor undertakes to deliver the goods to the Purchaser in the form of a commission supplement, as mentioned above, and to transfer the ownership of the goods to the Purchaser at the time of their collection by the Purchaser from the commission, and the Purchaser undertakes to collect the goods and pay the agreed price for the goods collected from the commission. – applies to tasks 2-10, 11 item 1, 12-22. 3. The Contractor should offer the subject of the contract in accordance with the parameters specified by the Purchaser. Where the Purchaser has not directly specified a dimension or other parameter, this is at the discretion of the Contractor. 4. The Purchaser guarantees that the minimum value of deliveries in the basic scope will be 60% of the maximum value of that task in the basic scope specified in the Price List constituting Annex 1 to the Contract. The Purchaser informs that in the subject procedure conducted in the unrestricted tendering procedure, it envisages the possibility of applying the procedure referred to in Article 139(1) of the p.z.p. act. When applying the mentioned procedure, the Purchaser will first carry out an examination and evaluation of the offers, and then carry out a subjective qualification of the Contractor, whose offer was highest rated in terms of the lack of grounds for exclusion and the fulfillment of the conditions for participation in the procedure. In terms of non-exclusion, each of the Contractors jointly applying for the award of the contract demonstrates the lack of grounds for exclusion separately, by referring to their own certificate of non-exclusion or by submitting the subjective evidence required in the SWZ. Declarations of belonging or non-belonging to the same capital group are made by each of the Contractors jointly applying for the award of the contract, regardless of the use of a certificate.
