Poland – Tomography devices – Zakup i dostawa bezwkładowego automatycznego wstrzykiwacza do tomografii komputerowej wraz z instalacją i montażem urządzenia dla potrzeb Katowickiego Centrum Onkologii.

1. The subject of the contract is the purchase and delivery of a non-contrast automatic injector for a CT scanner, including installation and assembly of the equipment for the needs of the Katowice Oncology Centre, in accordance with Annex No. 2 to the SWZ – Price Offer. 2. The technical and functional parameters of the equipment are specified in Annex No. 4 to the SWZ. The contract also includes: 1) transport of the equipment to the destination, 2) installation and assembly, 3) the cost of annual service inspections during the warranty period, 4) delivery within 4 weeks from the date of signing the contract, 5) a 36-month warranty counted from the date of signing by the authorized representatives of the Purchaser and the Contractor of the "Final Acceptance Protocol of the Contract Subject". 6) As part of the warranty, the Contractor guarantees the operability of the equipment in the offer price, including: • service inspections with all necessary parts for their performance, according to the manufacturer's requirements • all repairs, inspections, maintenance with spare parts and materials needed for their implementation, • repair and replacement of parts with new ones, except in cases where the damage is caused by improper use of the equipment, • extension of the warranty for the period of unavailability of the contract subject in the event of warranty repairs, • commencement of repair within 24 hours from the date of notification of the fault, from Monday to Friday, excluding public holidays, • repair time without the need to import parts from abroad: max. 3 days, from Monday to Friday, excluding public holidays, • repair time requiring the import of parts from abroad: max. 8 days, from Monday to Friday, excluding public holidays, • the total unavailability of the equipment (in the event of a fault) will not exceed 20 working days in 12-month periods * unavailability means the inability of the equipment to perform all procedures with its use for more than 5 hours a day, including faults mentioned in Annex No. 4 - Technical and Functional Parameters. 7) delivery of a factory-new, unused device, with a year of manufacture not earlier than 2025.
