Poland – Laboratory reagents – Dzierżawa analizatorów do posiewu krwi, identyfikacji i lekowrażliwości drobnoustrojów, barwienia metodą Grama, wykrywania prątków kwasoodpornych, diagnostyki metodą LAMP, ELISA, immunoblot i fluorescencji pośredniej wraz z niezbędnymi testami i akcesoriami

The subject of the contract is the lease of blood culture analysers, identification and antimicrobial susceptibility testing, Gram staining, detection of acid-fast bacilli, LAMP, ELISA, immunoblot and indirect immunofluorescence diagnostics, along with the necessary tests and accessories for a period of 24 months (see - assortment-price form, i.e. Subject of the Contract (SOC) - Annex 2 to SWZ) and in the Technical Parameters Description – Annex 3 to SWZ from the date of the contract award. 4. The documentary evidence required from the Contractor includes: 1) Information from the National Criminal Register in the scope of: a) art. 108(1)(1) and (2) of the Pzp Act, b) art. 108(1)(4) of the Pzp Act, regarding a court order prohibiting participation in public procurement as a criminal measure, issued no earlier than 6 months before its submission. 2) A declaration by the Contractor, under art. 108(1)(5) of the Pzp Act, of no affiliation with the same capital group, within the meaning of the Act of 12.07.2023 on the protection of competition and consumers (Dz. U. z 2025 r. poz. 1714), with another contractor who submitted a separate offer, partial offer or application for admission to the procedure, or a declaration of affiliation with the same capital group together with documents or information confirming the preparation of the offer, partial offer or application for admission to the procedure independently of another Contractor belonging to the same capital group - Annex 8 to SWZ, 3) Declarations by the Contractor of the currency of the information contained in the declaration, referred to in art. 125(1) of the Pzp Act, in the scope of the grounds for exclusion from the procedure indicated by the Contracting Authority, referred to in: a) art. 108(1)(3) of the Pzp Act, b) art. 108(1)(4) of the Pzp Act, concerning a court order prohibiting participation in public procurement as a preventive measure, c) art. 108(1)(5) of the Pzp Act, concerning the conclusion with other Contractors of an agreement aimed at disrupting competition, d) art. 108(1)(6) of the Pzp Act. - Annex 5 to SWZ. 4) Declarations by the Contractor in the scope of the grounds for exclusion from the procedure, referred to in art. 5k of Regulation (EU) 2022/576 of 8 April 2022 amending Regulation (EU) No 833/2014 concerning restrictive measures in view of Russia's actions destabilising the situation in Ukraine (OJ L 111/1 of 8.4.2022) and art. 7(1)(1-3) of the Act of 13 April 2022 on special solutions in the field of counteracting the support of aggression on Ukraine and serving to protect national security (Dz. U. z 2025 r., poz. 514). 5) Copy or information from the National Court Register or from the Central Register and Information on Business Activity, under art. 109(1)(4) of the Pzp Act, issued no earlier than 3 months before its submission, if separate provisions require registration in the register or record. 6) A list of supplies comparable to the supplies constituting the subject of the contract, carried out, and in the case of recurring or continuous services also carried out, in the last 3 years, and if the period of business activity is shorter - in that period, with the indication of their value, subject, date of performance and entities, for which the supplies were carried out or are being carried out, and the attachment of evidence specifying whether these supplies were carried out or are being carried out properly, whereby the evidence referred to are references or other documents issued by the entity, for which the supplies were carried out, and in the case of recurring or continuous services are carried out, and if the Contractor, for reasons independent of him, is not able to obtain these documents – a declaration by the Contractor; in the case of recurring or continuous services still being carried out, references or other documents confirming their proper performance should be issued within the last 3 months – in accordance with the condition set out in Section VIII. point 2.4) - Annex 9 to SWZ, To confirm that the Contractor meets the conditions for participation in the procedure: List of supplies mentioned in Section VIII. CONDITIONS FOR PARTICIPATION IN THE PROCEDURE para. 2 point 3) The Contractor is obliged to attach to the offer a preliminary declaration in the form of a single document (part IV section α) – Annex 4 to SWZ, and the one whose offer is highest rated, upon request of the Contracting Authority, also documents confirming these circumstances: The Contractor meets the condition if it proves that in the last 3 years, and if the period of business activity is shorter - in that period, it has properly carried out at least 3 supplies consisting of supplies comparable to the supplies constituting the subject of the contract with a minimum value for individual packages: Package 1 A / 1 B 128 108,00 Package 2 A 73 031,00 Package 2 B 19 256,00 Package 3 135 957,00 Package 4 138 452,00 Package 5 79 857,00 Package 6 29 270,00 gross each supply. A Contractor submitting an offer for several packages must prove that it has carried out, and in the case of continuous services, also carries out, a minimum of 3 supplies of the required total value equal to the sum of those packages, to which the offer relates.
