The subject of the contract is: Catering service for the preparation and distribution of meals, including the rental of a kitchen facility for SPZOZ WSS Nr 3 in Rybnik, hereinafter referred to as the Contracting Authority. The contract will be awarded for a period of 36 months following the completion of the procedure. The Contracting Authority envisages the possibility of changes to the provisions of the concluded contract (so-called contractual changes based on art. 455, para. 1, points 1 and 2 of the Pzp) in relation to the content of the offer on the basis of which the Contractor was selected, in accordance with the planned provisions of the contract - conditions set out in Annex No. 2 to the SWZ. The contract may also be changed in cases referred to in art. 455, para. 1, points 3 and 4 and para. 2 of the Pzp. A detailed description of the subject of the contract and the conditions for the implementation of the contract, which form the basis for the calculation of the offer price, are contained in the contract documents - i.e., the Specification of Contract Terms (hereinafter SWZ) and the annexes. The Contracting Authority informs that, in accordance with art. 139 of the Pzp, it will first carry out an examination and evaluation of the offers, and then carry out a substantive qualification of the Contractor whose offer was highest rated, in terms of the absence of grounds for exclusion and the fulfillment of the conditions for participation in the procedure. In fulfilling the obligation arising from art. 24, para. 6 of the Act of 14 June 2024 on the protection of whistleblowers, the Contracting Authority informs that in the Samodzielny Publiczny Zakład Opieki Zdrowotnej Wojewódzki Szpital Specjalistyczny Nr 3 in Rybnik, a procedure for internal reporting of information about breaches of law and the taking of follow-up actions has been adopted.
