Poland – Dressings – Zakup i dostawa środków opatrunkowych

1. The subject of the contract is the successive delivery of dressings. 2. The subject of the contract is described in Annex 2.1 – 2.4 to the SWZ (price list) (contract part/package/task no. 1 – 4) – also jointly referred to as “Annex 2 to the SWZ”. 3. The place of performance of the contract: ul. Grenadierów 51/59, 04-073 Warsaw. 4. The successive deliveries of the subject of the contract are to be made to the Hospital Pharmacy located in Pavilion III of the Hospital on the ground floor and to the warehouse located in the basement of the Hospital, with the Contracting Authority indicating each time if the delivery place will be the warehouse. 5. The acceptance of the subject of the contract will take place in the premises accessible to the Contractor's (suppliers', couriers') staff. 6. The detailed terms of the service are specified in the contract project constituting Annex 4 to the SWZ. 7. The Contracting Authority allows for the submission of partial offers (4. - task / part of the contract), in accordance with the division specified in Annex 2 to the SWZ constituting the Description of the Subject of the Contract: 1) task no. 1 – Haemostatic and bactericidal dressings; 2) task no. 2 – Sterile dressings; 3) task no. 3 – Healing balm; 4) task no. 4 – Hydrogel and Manuka honey dressings. 8. The individual parts of the contract will be performed independently of each other.
