The subject of the contract is: Supply of implantable medical devices Task 1. Three-chamber resynchronization pacemaker with accessories along with programmer rental Task 2. Single- and dual-chamber pacemaker with accessories along with programmer rental Task 3. Single-chamber cardioverter-defibrillator ICD VR with atrial signal detection with accessories along with programmer rental Task 4. Cardioverter-defibrillator VR, DR and CRT-D with accessories Task 5. Cardioverter-defibrillator VR, DR and CRT-D with smartphone monitoring capability Task 6. Single-chamber cardioverter-defibrillator VR and DR with reduced volume with accessories along with programmer rental Task 7. Three-chamber resynchronization pacemaker with longer battery life with accessories Task 8. Single-chamber cardioverter-defibrillator ICD VR with smartphone monitoring capability and increased defibrillation energy The contracting authority allows the submission of partial offers. The parts of the contract are the individual tasks specified above, i.e. tasks 1 to 8. The contracting authority informs that the Contractor may submit an offer for a maximum of 8 tasks (without limiting the number of tasks per Contractor). The detailed description of the subject of the contract is contained in Annexes 2b and 4 to the SWZ.
