Description

I. The subject of the contract is the supply of a set for liquid chromatography UHPLC for pressures of 130 MPa with a high-pressure gradient and a triple quadrupole mass spectrometer – 1 set. The detailed description and the method of executing the contract are contained in the assortment-price table Annex No. 2 to the SWZ, the description of the subject of the contract ("OPZ") Annex No. 3 to the SWZ and the contract template Annex No. 4 to the SWZ. II. The contracting authority does not allow for the possibility of submitting partial offers. The set, as a technical and functional whole, serves for ultra-sensitive, precise quantitative and qualitative tests in clinical and medical laboratories. It allows for ultra-fast separation of complex biological samples and certain identification of the molecules tested. III. In accordance with Article 13(1) and (2) and Article 14(1) and (2) of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) (OJ EU. L. 2016, No. 119, p. 1) ("GDPR"), we would like to inform you about the principles of processing personal data in the event that the contracting authority processes your personal data in connection with the conduct of the public procurement procedure. 2. The administrator of your personal data is the contracting authority. 3. The contracting authority can be contacted in matters related to the protection of personal data: 1) by sending a letter to the address of its seat or electronically via e-mail correspondence to the address: [email protected] or 2) via the ePUAP platform to the address: /IMiDWarszawa/SkrytkaESP. 4. The contracting authority has appointed a Data Protection Officer ("DPO"), with whom you can contact regarding the processing of your personal data. Contact with the DPO is possible via e-mail at: telephone iod@imid. nr (22) 32 77 495 or by mail to the address of the contracting authority. 5. The contracting authority processes your personal data for the following purposes: 1) conducting the public procurement procedure – on the basis of Article 6(1)(c) GDPR in conjunction with Article 9(2)(g) GDPR in conjunction with the provisions of the Public Procurement Law; 2) verification of information on non-criminality within the framework of evidential means of the entity – on the basis of Article 6(1)(c) in conjunction with Article 10 GDPR in conjunction with the provisions of the Public Procurement Law; 3) archiving personal data – on the basis of Article 6(1)(c) GDPR in conjunction with Article 9(2)(j) GDPR in conjunction with the Act on the National Archival Resource and Archives; 4) establishing, pursuing or defending claims in connection with the conducted activity – on the basis of Article 6(1)(e) GDPR or Article 9(2)(f) GDPR, i.e. processing is necessary for the performance of a task carried out in the public interest – proper management of the contracting authority's property, i.e. conducting an important public procurement procedure, guaranteeing the correct expenditure of public funds. The detailed GDPR information clause has been included in the SWZ. IV. The subject of the contract is carried out within the health policy programme: "Government Programme for Newborn Screening Tests for 2019-2026".
