The subject of the contract is the successive supply of reagents for performing diagnostic tests along with the rental of necessary equipment for performing these tests for the needs of the Medical Diagnostic Laboratory SPZOZ dr. J.Dietla Hospital in Krynica-Zdroju for a period of 36 months from the date of signing the contract, in accordance with the description and requirements set out in Annex 2 to the SWZ. 2. The remaining terms of the contract are specified in the contract project, which is Annex 3 to the Specification. 3. The contract has been divided into 12 parts/packages.
