The subject of the contract is the delivery to the Wojewódzki Hospital in Bielsko-Biała at Al. Armii Krajowej 101, of laboratory reagents and consumable materials according to the quantitative and qualitative list specified in Annexes No. 2.1–2.3 to the Specification of Contract Terms, hereinafter referred to as "swz", together with the lease of analysers, meeting the operational and user parameters specified in Annexes No. 2.1 – 2.3 to swz, on the conditions specified in Annexes No. 2.1–2.3 and the draft provisions of the contract constituting Annex No. 5 to swz. 2. The offered laboratory reagents and consumable materials must be new and unused. 3. The delivery of the subject of the contract, including transport and unloading, will be at the cost and risk of the Contractor.
