The subject of the procurement is group life insurance for employees. 2) The detailed specification of the subject of the procurement is set out in Part II of the SWZ and Part IV of the SWZ. 3) In accordance with the provisions of 438 of the Act in connection with Article 95(1) of the Act, the requirements regarding the obligation to employ workers are specified in Part II of the SWZ and Part IV of the SWZ. 4) The Contracting Authority requires the Contractor to indicate in the tender, which parts of the procurement the Contractor intends to award to Subcontractors and for the Contractor to provide the names of the Subcontractors, if they are already known. 5) The Contracting Authority does not reserve the right to require the Contractor to personally perform key tasks related to the services. 6) Name and code of the Common Procurement Vocabulary: Code: 66511000-5 Life insurance services. 7) The Contracting Authority does not require the submission of subject-related evidentiary means with the tender. 8) Acting under Article 133(3) of the Act, the Contracting Authority establishes requirements regarding the confidentiality of information provided to Contractors during the procedure. The following parts of the SWZ are confidential: Part II of the SWZ – Description of the subject of the procurement. Part IV of the SWZ – Draft contractual provisions. 9) Pursuant to Article 133(3) of the Act, the Contracting Authority does not make the contents mentioned in point 8 publicly available on the internet site due to the protection of the confidential nature of the information contained in the SWZ. The files named “Part II of the SWZ – Description of the subject of the procurement” and “Part IV of the SWZ – Draft contractual provisions” uploaded to the procurement platform have been password-protected. In order to protect the confidential nature of the Confidential Information provided during this procedure, the Contracting Authority reserves the right that the password to the confidential information referred to above will be provided after the Contractor submits a confidentiality undertaking signed with a qualified electronic signature. It is required to submit the undertaking by persons authorised to represent the Contractor (this authorisation must result from the registry documents or the power of attorney attached to the submitted undertaking) and by persons who will participate in the procedure. The templates for the confidentiality undertakings are Annexes No. 3.1 and 3.2 to Part III of the SWZ. The Contracting Authority informs that letters with explanations of the contents of the SWZ relating to the confidential part of the procurement (if any are submitted during the conduct of the procedure) will also be published in an encrypted form – to decrypt, the above-mentioned password will be used (this password will be used throughout the conduct of the entire procedure). The declarations should be submitted via the Procurement Platform, tab “Questions/Information”. After their receipt and verification, the password for decrypting the documents will be sent to the e-mail address indicated in the information about the transmission of the password for decrypting the documents. Note: If the persons who will participate in the procedure do not have qualified electronic signatures, the Contracting Authority allows these declarations to be signed manually by the persons submitting the declarations, scanned, confirmed as consistent with the original by persons authorised to submit declarations of will and knowledge in the name of the Contractor and in such form submitted to the Contracting Authority via the Procurement Platform.
