Zamowlenie on the subject of the supply of reagents, test strips, operational materials and consumables, calibrators, control materials, the provision of service services and the lease of laboratory analyzers with additional equipment for tests in the field of hematology, immunochemistry, coagulology, critical parameters (RKZ) and urine analysis. The procedure has been divided into 5 parts. A detailed description of the subject of the order are appendices 2.1-2.5 and 3.1-3.5 to SWZ.

1. Information of the Personal Data Administrator, pursuant to art. 13 para. 1 and 2 of the General Data Protection Regulation no 2016/679 of 27 April 2016 (hereinafter GDPR). Informs that:

Gliwicka 5, whose registry file is kept by the District Court in Gliwice, X Economic Department of the National Court Register under number 0000044032, REGON: 000293114, NIP: 9690946632 (hereinafter: Psychiatric Hospital in Toszek), email: 2) The Data Protection Inspector has been appointed [email protected], www.szpitaltoszek.pl; Mr. Paweł Wojciechowski - email contact: [email protected] - in any matter related to the processing of your personal data; 3) Your personal data will be processed on the basis of art. 6 para. 1 lit. c GDPR for the purpose related to the subject matter of the procedure for awarding a public contract 4) the recipients of your personal data will be persons or entities to whom the documentation of the procedure will be made available on the basis of art. 74 of the Pzp; 5) Your personal data will be stored, in accordance with art. 78 para. 1 Pzp for a period of 4 years from the date of completion of the procedure for awarding the contract, and if the duration of the contract exceeds 4 years, the storage period covers the entire duration of the contract; 6) the obligation to provide your personal data directly concerning you is a legal requirement specified in the provisions of the Pzp, related to participation in the procedure for awarding a public contract. 7) in relation to your personal data, decisions will not be made in an automated manner, in accordance with art. 22 GDPR. 8) you have: a) the right, pursuant to art. 15 GDPR, to access your personal data (in the event that exercising this right would require disproportionately large effort on the part of the administrator, you may be obliged to indicate additional information aimed at specifying the request, in particular by providing the name or date of the public procurement or tender procedure or specifying the name or date of the completed public procurement procedure); b) the right, pursuant to art. 16 GDPR, to rectify your personal data (exercising the right to rectification may not result in a change in the result of the public procurement procedure or a change in the provisions of the contract in a manner inconsistent with the Pzp and may not violate the integrity of the protocol and its annexes); c) the right, pursuant to art. 18 GDPR, to request the administrator to restrict the processing of personal data with the reservation of the period of the public procurement procedure or tender and cases referred to in art. 18 para. 2 GDPR (the right to restrict processing does not apply to storage, for the purpose of ensuring the use of legal protection means or for the purpose of protecting the rights of another natural or legal person, or due to important public interest reasons of the European Union or a Member State); d) the right to lodge a complaint with the President of the Personal Data Protection Office, if you consider that the processing of your personal data violates the provisions of the GDPR; 9) you do not have: a) the right to erasure of personal data, pursuant to art. 17 para. 3 lit. b, d or e GDPR; b) the right to data portability, referred to in art. 20 GDPR; c) the right to object, pursuant to art. 21 GDPR, to the processing of personal data, as the legal basis for the processing of your personal data is art. 6 para. 1 lit. c GDPR; 10) you have the right to lodge a complaint with the supervisory authority regarding the processing of your personal data by the administrator that is not in accordance with the GDPR.
