In the case of medicines, special dietary foods and medical devices covered by reimbursement, the prices offered and applied by the Contractor may not exceed the maximum prices determined on the basis of Article 9(1) of the Act of 12 May 2011 on the reimbursement of medicines, special dietary foods and medical devices. The unit packaging of the product must contain visible CE marking, batch number, expiry date and single-use mark, the size and quantity of items in the packaging must be visible on the packaging. A detailed description of the subject of the order is specified in the assortment-price form - attachment No. 2 to SWZ. The Purchaser allows other packaging sizes, converted to full packages (rounded up). The Contractor is obliged to implement the order on the terms and conditions described in the contract template constituting attachment No. 4 to SWZ.
