[Numeration in accordance with the SWZ] 4.1 The subject of the contract is the successive supply of consumable materials for printers, copiers, and multifunctional devices in 2027. 4.2 The detailed description of the subject of the contract is in Annex No. 1 to the SWZ and in Annex No. 5 to the SWZ. 6. CONTRACT PERFORMANCE TERM 6.1 Contract performance term: from the date of signature but no earlier than 04.01.2027 to 31.12.2027 or until the contract amount is exhausted, whichever occurs first 7. GROUNDS FOR EXCLUSION FROM THE PUBLIC PROCUREMENT PROCEDURE are described in point 7 of the SWZ. 8. INFORMATION ON PARTICIPATION CONDITIONS IN THE PROCEDURE FOR AWARDING THE CONTRACT 8.1 Contractors who meet the following conditions may apply for the contract: 8.1.1 capacity to operate in the economic market: The Contracting Authority does not specify any requirements in this regard, the fulfilment of which the Contractor is obliged to demonstrate in a special manner. 8.1.2 authorisations to carry out a specific economic or professional activity, if this results from separate provisions: The Contracting Authority does not specify any requirements in this regard, the fulfilment of which the Contractor is obliged to demonstrate in a special manner. 8.1.3 economic or financial situation: The Contracting Authority does not specify any requirements in this regard, the fulfilment of which the Contractor is obliged to demonstrate in a special manner 8.1.4 technical or professional capacity: The Contracting Authority does not specify any requirements in this regard, the fulfilment of which the Contractor is obliged to demonstrate in a special manner JEDZ, LIST OF SUBJECTIVE EVIDENTIAL MEANS AS WELL AS OTHER DOCUMENTS In accordance with Article 139(1) of the Pzp Act, the Contracting Authority will first examine and evaluate the offers, and then qualify the Contractor, whose offer was most highly evaluated, in terms of the absence of grounds for exclusion and the fulfilment of the conditions for participation in the procedure. 9.1 List of declarations and documents which the Contractor submits together with the offer: 9.2 The Contractor is obliged to submit, together with the offer, in order to preliminarily confirm that it is not subject to exclusion and meets the conditions for participation in the procedure: 9.2.1 Declaration of the Contractor regarding the grounds for exclusion from art. 5k of Regulation 833/2014 in the wording given by Regulation 2022/576 and art. 7(1) of the Act on special solutions in the field of counteracting support for aggression on Ukraine and serving to protect national security, submitted on the basis of Article 125(1) of the Pzp Act, in accordance with the template constituting Annex No. 7 to the SWZ. In the case of contractors jointly applying for the contract, this declaration is submitted by each of these contractors (this also applies to partners in a civil law partnership). 9.9 Information regarding certification is in point 9.9 of the SWZ 15 METHOD OF SUBMITTING OFFERS The offer consists of the following documents: 15.1 Offer form – prepared according to the template - Annex No. 2 to the SWZ (signed with a qualified electronic signature). 15.2 Price form calculation sheet file Annex No. 2a to the SWZ - (signed with a qualified electronic signature). Note: 1. Not attaching to the offer Annex No. 2a to the SWZ price form, not filling in all its positions necessary to calculate the price or not signing the aforementioned annex with a qualified electronic signature - will result in the offer being rejected by the Contracting Authority. 2.
