1. The subject of the order is: Supplies of orthopaedic medical materials. The order is divided into 11 packages (parts). Contracts in respect of the resolved packages will be concluded after the completion of this procedure for a period of 12 months. 2. The detailed description of the subject of the order is contained in the annexes: 1) Price lists (separately for each of the packages) – Annex No. 1.1 to SWZ, 2) Draft contractual provisions, which will be incorporated into the contract / contractual provisions – Annex No. 2 to SWZ. 3. The ordering party informs that, in accordance with Article 139 of the PZP, it will first carry out an examination and evaluation of the offers, and then carry out a subjective qualification of the Contractor, whose offer was most highly evaluated, in terms of the lack of grounds for exclusion and the fulfilment of the conditions for participation in the procedure. 4. In fulfilling the obligation arising from Article 24(6) of the Act of 14 June 2024 on the protection of whistleblowers, the ordering party informs that in the Samodzielny Publiczny Zakład Opieki Zdrowotnej Wojewódzkim Szpitalu Specjalistycznym Nr 3 in Rybnik, a procedure for internal notifications of information about breaches of the law and subsequent actions has been adopted.
