Poland – Reagents and contrast media – 213/PN/2026 - Dostawa środków kontrastowych dla UCK

1. The subject of the contract is the supply of contrast media for UCK in the assortment, estimated quantity and requirements specified in Annex 3 to the SWZ. 2. The contracting authority allows for the submission of partial offers. By partial offer, it is understood to be individual items set out in Annex 3 to the SWZ. The contractor may submit an offer for one or more items. 3. The contracting authority has specified the method of packaging in the assortment-price form. In positions where there is no such description, the contractor may offer any method of packaging while maintaining the quantity required by the contracting authority specified in the given position. The contractor must price the proposed method of packaging with the appropriate conversion. 4. Deliveries will follow successively, in quantity and assortment, in accordance with the partial orders of the contracting authority within 2 days from the date of the order, and delivery "immediately" within 12 hours from the notification of the order. The ordered assortment the contractor will be obliged to deliver at their own cost to the Hospital Pharmacy. 5. The contracting authority requires that the prices of the drugs offered are not higher than the official prices, specified in art. 9 para. 1 and 2 of the Act of 12.05.2011 on the reimbursement of drugs, foodstuffs for special medical purposes and medical devices (Journal of Laws 2025 item 907), in force on the date of submission of offers - if applicable. 6. The offered medicinal products must be in accordance with the Act of 6 September 2001 on Pharmaceutical Law (Journal of Laws 2025 item 750) – if applicable. The offered medicinal products must be admitted for sale in Poland on the basis of art. 3 or 4a of the Pharmaceutical Law. 7. In cases where the contracting authority has used in the description of the subject of the contract designations of standards, technical assessments, technical specifications and technical reference systems, as referred to in art. 101 para. 1 point 2 and para. 3 of the Pzp, equivalent solutions are allowed. Each time a standard is indicated in this SWZ or in the annexes to the SWZ, it should be assumed that the formulation "or equivalent" has been used in relation to it. In cases where the description of the subject of the contract refers to standards, technical assessments, technical specifications and technical reference systems, as referred to in para. 1 point 2 and para. 3, the contracting authority may not reject an offer solely on the grounds that the offered supplies are not in accordance with the standards, technical assessments, technical specifications and technical reference systems to which the description of the subject of the contract refers, provided that the contractor proves in the offer, in particular by means of the evidential means referred to in art. 104-107, that the proposed solutions meet the requirements specified in the description of the subject of the contract to an equivalent degree. 8. The contracting authority requires the submission of offers in electronic form. A detailed description (instructions) for the submission of offers in electronic form is contained in section XIV of the SWZ. The contracting authority informs that in the present procedure for the award of a public contract, the so-called "reverse order of evaluation of offers" will be applied, as referred to in art. 139 of the PZP. The contracting authority, by applying the above procedure, will first carry out an examination and evaluation of the offers, and then carry out a subjective qualification of the contractor whose offer was most highly evaluated in terms of the absence of grounds for exclusion and compliance with the conditions of participation in the procedure. 9. The contractor may entrust the execution of part of the contract to a subcontractor. 10. The contracting authority does not allow the submission of variant offers. 11.
