Poland – Medical practice and related services – Świadczenie usług medycznych z zakresu dodatkowej opieki medycznej dla pracowników spółki Aplikacje Krytyczne Sp. z o.o. i ich rodzin

The subject of the contract is the provision of medical services in the form of additional medical care for the employees of Aplikacje Krytyczne Sp. z o.o. and their families, in the following packages: 1) Individual Package I – employee of the Contracting Authority; 2) Partner Package II – employee of the Contracting Authority and a close relative of the employee; 3) Family Package III - employee of the Contracting Authority and close relatives of the employee; The Contracting Authority requires the Contractor to have a minimum of 230 facilities (owned or cooperating) in the Mazowieckie voivodeship and at least 1170 facilities in the remaining voivodeships of Poland, providing medical services. Facilities offering services only in the field of occupational medicine are not counted towards the number of facilities. The Contracting Authority reserves the right to verify the scope of activities of individual facilities. The Contractor is obligated to indicate the number of facilities in the list constituting an annex to the Offer Form. Failure to meet the above requirement will result in the rejection of the offer. The Contracting Authority envisages the possibility of exercising the right of option. Basic Contract: Provision of medical services in the field of additional medical care for the employees of Aplikacje Krytyczne Sp. z o.o. (Contracting Authority) and their families for a period of 36 months from the date of signing the contract. Contract under the right of option: Provision of medical services in the field of additional medical care for the employees of Aplikacje Krytyczne Sp. z o.o. (Contracting Authority) and their families for a period of up to 12 months from the date of completion of the contract. 2.
