The subject of the contract is the supply of multiplex PCR tests for the genetic diagnosis of infections on syndromic panels detecting viruses, bacteria, resistance mechanisms and fungi in the automated closed system FilmArray (manufacturer: BioFire / BioMerieux) owned by the Purchaser, along with the rental of 1 additional test module for the BioFire apparatus from the company Biomerieux and the supply of antibiotic concentration gradient strips for determining MIC, antibiotic resistance inhibition discs (1 vial = 50 discs = 50 identifications), reference strains for quality control from the ATCC collection, manual microbiological identification tests, immunochromatographic identification tests for detecting carbapenemases, divided into 5 parts. The Contractor may submit a bid for any part or parts or for the entire subject of the contract. The Purchaser does not allow the submission of variant bids. The Purchaser – pursuant to Article 139 of the Public Procurement Law – informs that, first of all, it will carry out an examination and evaluation of the bids in this procedure for the award of a public contract, and then it will qualify the Contractor whose bid has been most highly evaluated, in terms of the lack of grounds for exclusion and the fulfilment of the conditions for participation in the procedure.

DOCUMENTS TO BE SUBMITTED WITH THE BID

1. The bid consists of: 1) the completed tender form, in accordance with Annex No. 1 to the SWZ; 2) the completed assortment-price form, in accordance with Annex No. 2 to the SWZ.

2. With the bid, the Contractor is obliged to submit: 1) a copy or information from the National Court Register, Central Information and Business Activity Register or another appropriate register – in order to confirm that the person acting on behalf of the Contractor is authorized to represent it. The Contractor is not obliged to submit these documents if the Purchaser can obtain them through free and publicly available databases, provided that the Contractor has indicated the data enabling access to these documents (original); 2) if the person acting on behalf of the Contractor is not authorized to represent it, as evidenced by the documents referred to in the preceding point: a power of attorney or other document confirming the authorization to represent the Contractor; 3) a power of attorney to represent Contractors jointly applying for the award of the contract - if applicable; 4) in the case of submitting a bid bond in a form other than monetary – the original of the bid bond document (guarantee or surety) – if applicable. 5) Declaration and documents indicated in Section VI A to the extent specified therein as submitted with the bid.
