Poland – X-ray devices – Dostawa, instalacja w miejscu użytkowania, uruchomienie oraz przekazanie do użytku aparatu RTG na potrzeby realizacji zadania „Modernizacja i rozszerzenie działalności AOS wraz z zakupem sprzętu medycznego oraz uruchomienie POZ w Szpitalu Dziecięcym Polanki im. Macieja Płażyńskiego w Gdańsku sp. z o.o.”

The subject of the contract is the supply, installation on-site, commissioning and handover for use of an RTG apparatus for the purpose of implementing the task – Modernisation and expansion of AOS activities along with the purchase of medical equipment and the launch of POZ at Szpital Dziecięcy Polanki im. Macieja Płażyńskiego Sp. z o.o. in Gdańsk. 2. The detailed description of the subject of the contract is defined in the OPZ (Annex 2 to the SWZ), detailed description of parameters (Annex 3 to the SWZ) and in the contract template (Annex 4 to the SWZ).

CONDITIONS FOR PARTICIPATION IN THE PROCEDURE: regarding technical or professional capacity, i.e.: The contractor shall demonstrate that in the last 3 years before the deadline for submission of tenders, or if the period of business activity is shorter – in this period, it has completed at least three deliveries of medical equipment with a gross value of each not less than 700 000,00 PLN. NOTICE: i. In the event that the settlement for deliveries made by the contractor took place in a currency other than PLN, the contractor, in order to confirm compliance with the condition of participation in this procedure, is obliged to convert the value of the deliveries made, taking the average exchange rate of the zloty against foreign currencies specified in the table of average exchange rates of foreign currencies of the National Bank of Poland published on the day of the publication of the contract notice and the SWZ on the Platform. ii. In the event that the contractor has implemented the contract as part of a group of contractors (consortium), it may only refer to its own, real experience, which it has gained by implementing the part of the contract assigned to it within the group, i.e. it may demonstrate experience only in the scope in which the given entity actually performed. Experience is not gained through mere participation in the consortium, but through specific actions carried out within it. iii. If the scope of deliveries presented in the document submitted to confirm that the deliveries were made properly is wider than that specified by the Contracting Authority, the value of the deliveries confirming compliance with the condition of participation in the procedure should be indicated in the list of deliveries. iv. By 3 deliveries, we mean deliveries made on the basis of separate contracts. 2. When assessing technical or professional capacity, the contracting authority may, at any stage of the procedure, consider that the contractor does not have the required capacities if the contractor's conflicting interests, in particular the engagement of the contractor's technical or professional resources in other economic activities of the contractor, may have a negative impact on the implementation of the contract. 3. The contractor may, in order to confirm compliance with the conditions of participation in the procedure, as referred to in Section 1.2 of the SWZ, in appropriate situations and with regard to a specific contract, or part of it, rely on the technical or professional capacities of entities providing resources, regardless of the legal nature of the relationship linking them. 4. The contractor, who relies on the capacities or situation of entities providing resources, submits, together with the tender, a commitment of the entity providing resources to make the necessary resources available to it for the implementation of the given contract or another documentary evidence confirming that the contractor, when implementing the contract, will have the necessary resources of these entities at its disposal. 5. The commitment of the entity providing resources, as referred to in Section 4, confirms that the relationship linking the contractor with the entities providing resources guarantees real access to these resources and specifies in particular: 1) the scope of resources of the entity providing resources available to the contractor; 2) the method and period of making the resources of the entity providing these resources available to the contractor and their use by him when performing the contract; 3) whether and to what extent the entity providing resources, on the capacities of which the contractor relies in relation to the conditions of participation in the procedure regarding education, professional qualifications or experience, will carry out building works or services, the indicated capacities of which relate.
