Poland – Information systems and servers – Zakup sprzętu i oprogramowania wraz z wdrożeniem na potrzeby EZD i systemu zarządzania zasobami IT w ramach projektu pt. „Cyfrowa Policja – wdrożenie e-usług oraz systemu elektronicznego zarządzania dokumentacją w śląskiej Policji”

The subject of the order is the Purchase of equipment and software along with implementation for the needs of the IT Resource Management System and the EZD within the project entitled „Digital Police – implementation of e-services and an electronic document management system in the Silesian Police”. The detailed description of the subject of the order is Annex No. 3 to the SWZ. This order will be implemented from the funds of the project „Digital Police - implementation of e-services and an electronic document management system in the Silesian Police” within the European Funds for Silesia 2021-2027 (European Regional Development Fund) for the Priority: FESL.01.00-European Funds for intelligent development for the Action: FESL.01.04-Digitalisation of public administration. The ordering party, in accordance with Article 257 of the PZP Act, reserves the right to invalidate the procedure for awarding a public contract if the funds intended for the financing of the whole or part of the order have not been allocated to it. The offer is submitted under pain of invalidity in electronic form (i.e. with a qualified electronic signature). The offer should be prepared in accordance with the template constituting Annex No. 1 to the SWZ – offer form and Annex No. 1a to the SWZ – price form. The ordering party informs that Annex No. 1a to the SWZ, i.e. the price form, constitutes the content of the offer together with the offer form and is not subject to supplementation under Article 128 of the PZP Act. 1.5. The Contractor is obliged to attach to the offer: 1) a declaration, as referred to in Section IV, paragraph 1: point 1.2. a) – b) SWZ – in the form of JEDZ, and point 1.2. c) SWZ – in the template of Annex No. 5 to the SWZ. 2) a copy or information from the National Court Register, Central Register and Information on Business Activity or another appropriate register confirming that the person acting on behalf of the contractor is empowered to represent it, 3) a power of attorney or another document confirming the empowerment to represent the Contractor, if the empowerment of the person submitting the offer does not result from the documents described in point 4), 4) in the case of contractors jointly applying for the award of the contract: a) a power of attorney, in the case of contractors jointly applying for the award of the contract, from which it will result the empowerment to represent them in the procedure for the award of the contract or to represent in the procedure and conclude a contract in the matter of the public contract. The power of attorney should include in particular the indication: - the procedure for the public contract, to which it relates, - all contractors applying jointly for the award of the contract listed by name with the address of the registered office, - the appointed representative and the scope of his/her power of attorney, b) a declaration, as referred to in Section I, Section 4, paragraph 4.4 SWZ (the ordering party allows the submission of this declaration on the offer form); 5) a list of equivalent solutions along with their description, if the Contractor has applied equivalent solutions in accordance with Section 2, paragraph 2 SWZ 6) a commitment of the entity providing resources template constitutes Annex No. 11 to the SWZ (if applicable). In accordance with Article 126, paragraph 1 of the PZP Act, the ordering party, before selecting the most advantageous offer, will call on the contractor, whose offer has been most highly evaluated, to submit, within a specified time limit, not shorter than 10 days, current on the date of submission, subjective evidence or a certificate of the Contractor of public contracts replacing the appropriate subjective evidence. The certificate will be recognised by the ordering party only to the extent that it confirms the absence of grounds for exclusion or the fulfilment of the conditions of participation in the procedure specified in this SWZ. If the certificate does not cover all the circumstances, the Contractor is obliged to submit the remaining required subjective evidence. The subjective evidence has been indicated in the SWZ.
