The subject of the contract is: Tomotherapy accelerator with bunker modernization. The subject of the contract includes: 1) purchase by the Contracting Authority and sale by the Contractor of a brand-new tomotherapy accelerator (hereinafter referred to as the "equipment") 2) preparation of design documentation and construction and installation works related to the installation of the equipment and adaptation of the room at the Contracting Authority's Radiotherapy Department I, in order to meet functional and technical requirements, execution of all works within the scope of adaptation and renovation works covered by the subject of the contract 3) dismantling, removal, transport and disposal of the equipment, owned by the Contracting Authority, in accordance with applicable legal regulations, along with the transfer to the Contracting Authority of documents confirming the proper management of waste 4) delivery, installation, commissioning and handover of the equipment for exploitation; 5) conducting training for a minimum of 6 employees (in accordance with the SWZ) of the Contracting Authority, covering the operation of the equipment. The training will cover the medical personnel and engineering-technical personnel indicated by the Contracting Authority. The conducted training will be documented with named certificates and a list of trained persons signed by the training participants and their organiser. 2. The detailed description of the subject of the contract is contained in Annex No. 2 to the SWZ (description of the subject of the contract / price list) and Annex No. 9 to the SWZ (Functional-User Program).
