Poland – Construction work – Modernizacja 4 łazienek/toalet w ramach Projektu grantowego pn. „ Wsparcie infrastrukturalne Centrów Zdrowia Psychicznego dla dorosłych oraz ośrodków / zespołów środowiskowej opieki psychologicznej i psychoterapeutycznej (I poziom referencyjny)”.

The subject of the order is the implementation of the statement entitled: "Modernization of 4 bathrooms/toilets as part of the Grant Project entitled 'Infrastructure Support for Adult Mental Health Centres and environmental psychological and psychotherapeutic care centres/teams (first reference level)'. 2. The investment is carried out as part of the Grant Project No. FENX.06.01-IP.03-0007/24 entitled 'Infrastructure Support for Adult Mental Health Centres and environmental psychological and psychotherapeutic care centres/teams (first reference level)', implemented as part of the European Funds for Infrastructure, Climate, Environment 2021-2027, co-financed from the European Regional Development Fund, implemented on the basis of the Decision on co-financing the Project No.: FENX.06.01-IP.03-0007/24-00 adopted by the Minister of Health on 28 February 2025. Grant agreement No. U-30397/2026/00/305/2026/258 to the grant application No. W-30397. 3. A detailed description of the subject of the order is contained in separate annexes – the file DESCRIPTION OF THE SUBJECT OF THE ORDER (OPZ) in part B of the SWZ. The place of performance of the Order is: Independent Public Health Care Institution Central Clinical Hospital of the Medical University of Łódź with its registered office in Łódź at ul. Pomorska 251, 92-213 Łódź, location: PSYCHIATRY, building B-1, ul. Czechosłowacka 8/10, 92-216 Łódź. 4. Designation according to the Common Procurement Vocabulary (CPV): 45000000-0 Construction work; 45215140-0 Construction work in the field of hospital facilities; 45400000-1 Finishing work in the field of construction facilities - additional CPV in OPZ. 5. If specific technical solutions are indicated in the description of the subject of the order, equivalent solutions may be used, in terms of their characteristics and parameters – defined for a given position of the subject of the order, and all possible trade names of devices and products used in the description of the subject of the order should be treated as standard definitions, and not as specific trade names of devices, products used in this documentation. The obligation to prove equivalence lies with the Contractor. 6. The Purchaser requires, as referred to in Article 95(1) of the Act, i.e. employment by the Contractor or subcontractor of persons performing activities during the implementation of the order on the basis of an employment contract in the field of construction work implementation - detailed information is contained in the SWZ. 7. In the case of this procedure, the Purchaser has waived the division into parts because it considered that there is no possibility of dividing the order into parts. The Purchaser did not divide the order into parts due to the nature of the order (further in the SWZ). 8. The order will be implemented within a maximum period until 31.03.2027. 9. The scope and conditions for changes are contained in the model contract, which is an annex to the SWZ. 10. The remaining provisions as per the SWZ.
