The subject of the contract is the successive deliveries of single-use assortment in the type and quantities given in the price form constituting Annex 2 to the SWZ and in accordance with the planned provisions of the contract constituting Annex 4 to this SWZ, i.e. according to the tasks as follows: Task 1: Deliveries of vaginal speculum brushes Task 2: Deliveries of gastrostomy tubes Task 3: Deliveries of urological catheters Task 4: Deliveries of skull plate stabilisation clips Task 5: Deliveries of tonsil loops Task 6: Deliveries of Crawford-type intubation sets. Task 7: Deliveries of intravenous infusion cannulas Task 8: Deliveries of specialist intubation tubes Task 9: Deliveries of chest drainage tubes for the pump. Task 10: Deliveries of anaesthesiological accessories Task 11: Deliveries of jejunostomy sets, clips and adapters Task 12: Deliveries of nitrile gloves Task 13: Deliveries of biopsy and grasping forceps Task 14: Deliveries of Veress needles Task 15: Deliveries of titanium vascular clips for the Horizon clip applier Task 16: Deliveries of skin markers Task 17: Deliveries of stoma bags. Task 18: Deliveries of aluminium nasal stabiliser Task 19: Deliveries of suction tubes and holders Task 20: Deliveries of laparoscopic bags Task 21: Deliveries of endotracheal intubation tubes Task 22: Deliveries of cyanoacrylate synthetic glue Task 23: Deliveries of bags for preparing mixtures for parenteral nutrition Task 24: Deliveries of tissue specimen bags for laparoscopy Task 25: Deliveries of basins Task 26: Deliveries of patient care bowls Task 27: Deliveries of deep basins, deep basin covers and plastic supports for the deep basin Task 28: Deliveries of shallow basins, shallow basin covers and plastic supports for the shallow basin. 2. The contracting authority guarantees that the minimum value of orders awarded to the Contractor during the contract period in the basic scope will be 70% of the Contractor's remuneration for the given task in the basic scope, as specified in the Price Form constituting Annex 1 to the contract – the underlined entry applies to tasks 5, 6, 7. 3. The contracting authority is entitled to exercise the option right specified in art. 441 of the Public Procurement Law (hereinafter: "option right"), i.e. the quantitative option consisting in the possibility of increasing the quantity of the goods covered by the subject of the contract in relation to the quantity given in Annex 1 to the contract (price form) in the column "Quantity pcs. (basic scope)" by a maximum of the quantity specified in Annex 1 to the contract (price form) in the column "Quantity pcs. (option right)" – applies to tasks no. 5, 6, 7. The contracting authority informs that in the current procedure conducted in the open tender procedure, it envisages the possibility of applying the procedure referred to in art. 139(1) of the p.z.p. Act. When applying the above procedure, the contracting authority will first carry out an examination and evaluation of the offers, and then carry out a substantive qualification of the Contractor, whose offer was rated highest in terms of the lack of grounds for exclusion and the fulfilment of the conditions for participation in the procedure. The Contractor is obliged to provide security for the entire period of the tender specified in the text of this SWZ. 3. The offer of a Contractor who does not provide security, or provides security in an incorrect manner, or does not maintain security continuously until the expiry of the tender period or submits an application for the return of security in the case referred to in art. 98(2)(3) of the p.z.p. Act, will be rejected on the basis of art. 226(1)(14) of the Act. 4. Security may be provided in one or more of the following forms: a) cash; b) bank guarantees; c) insurance guarantees; d) guarantees given by entities referred to in art. 6b(5)(2) of the Act of 9 November 2000 on the establishment of the Polish Agency for Enterprise Development (Journal of Laws of 2025, item 98). 5. In the case of providing security in the form specified in para. 4 points 2-4 of the SWZ, it should result from its content the unconditional, irrevocable and upon the first written request submitted by the contracting authority (within the tender period) obligation of the Guarantor to pay the contracting authority the full amount of the security in the circumstances specified in art. 98(6
