Poland – CT scanners – 228/PN/2026 Dostawa sprzętu medycznego wraz z wykonaniem prac adaptacyjnych dla UCK.
Description
The subject of the contract is: 1) Delivery of medical equipment with the technical parameters specified in Annex 4a, 4b, 4c and 4d to the SWZ, hereinafter referred to as “equipment”, along with installation, commissioning and training of the Contracting Authority's staff; 2) Delivery of complete sets of single-use medical devices, in accordance with the requirements specified in Annex 18 to the SWZ; 3) Performance of adaptation works specified in Annex 16a, 16b, 16c and 16d to the SWZ. 2. The subject of this contract includes the following tasks: Task 1: 1) Operative computed tomography scanner (1 unit) with technical parameters specified in Annex 4a to the SWZ; 2) Complete sets of single-use medical devices necessary to perform procedures using a navigation system in operations performed under computer tomography control in accordance with the requirements specified in Annex 18 to the SWZ; 3) Performance of adaptation works in accordance with the requirements specified in Annex 16a to the SWZ; Task 2: 1) Biplane angiograph (2 units) with technical parameters specified in Annex 4b to the SWZ; 2) Performance of adaptation works in accordance with the requirements specified in Annex 16b to the SWZ; Task 3: 1) Stationary X-ray apparatus (1 unit) with technical parameters specified in Annex 4c to the SWZ; 2) Performance of adaptation works in accordance with the requirements specified in Annex 16c to the SWZ; Task 4: 1) Ceiling-mounted X-ray apparatus (1 unit) with technical parameters specified in Annex 4d to the SWZ; 2) Performance of adaptation works in accordance with the requirements specified in Annex 16d to the SWZ; 3. The document “Teletechniczny standard technologiczny UCK” containing data on central installations/systems installed in the Uniwersyteckim Centrum Klinicznym will be made available to Contractors in electronic form via the Platform conducting the procedure after the prior conclusion of a confidentiality agreement constituting Annex 17 to the SWZ. 4. The scope of works and all necessary documents necessary for the performance of adaptation works are in Annex 16a, 16b, 16c and 16d to the SWZ. 5. The equipment mentioned in point 2 must meet the boundary conditions specified respectively in Annex 4a, 4b, 4c and 4d to the SWZ. Failure to meet even one of the boundary conditions specified respectively in Annex 4a, 4b, 4c and 4d to the SWZ will result in the rejection of the offer. 6. The equipment mentioned in point 2 will be delivered with the necessary accessories for its proper functioning. 7. Delivery of the equipment referred to in point 2 will take place at the location indicated by the Contracting Authority in Gdańsk, at ul. Smoluchowskiego 17, at the Contractor's expense. 8. The Contractor will train the Contracting Authority's staff in the basics of equipment operation. 9. The Contractor will guarantee that the delivered equipment is factory new, complete, of high standard in terms of both quality and functionality, and free from material, structural and legal defects, and approved for use in public health care facilities. 10. The offered equipment must be factory new, produced in 2026 or 2027. 11. The Contracting Authority requires Contractors to conduct a local visit, on pain of rejection of the offer under art. 226 para. 1 point 18 of the PZP Act. The local visit will take place at the Uniwersyteckim Centrum Klinicznym, building CMI (Centrum Medycyny Inwazyjnej) I floor, registration TK/MR at the entrance to the Secretary of the Head of Radiology at ul. Smoluchowskiego 17 in Gdańsk on 04.09.2026 at 12:00. 12. To confirm the visit, a representative of the Contractor will sign a declaration of its completion during the visit – according to Annex 19 to the SWZ. 13. The Contracting Authority requires that the offered sets of single-use medical devices, referred to in point 2 sub-point 2) within the scope of Task 1, meet the requirements regarding medical devices contained in the Act of 7 April 2022 on medical devices (Journal of Laws of 2024 item 1620) and in the executive acts issued on the basis of this Act. 15. The sets of single-use medical devices offered within the scope of Task 1 must meet the absolute requirements specified in Annex 18 to the SWZ. Failure to meet even one of the absolute requirements specified in Annex 18 to the SWZ will result in the rejection of the offer. 16. Delivery of sets of single-use medical devices will be carried out on the basis of partial orders. The maximum delivery time is 6 working days from the date of the order placed by the Contracting Authority. 17. The Contractor will be obliged to deliver the sets of single-use medical devices to the Medical Supply Department at ul. Smoluchowskiego 17 at his own expense. 18. The subject of the contract within the scope of Task 1, 2 and 3 is co-financed from the funds of the Medical Fund within the targeted grant awarded by the State Treasury – Minister of Health on the basis of the agreement no. DOI/FM/SIS/ONKO/2026/25/181 for the co-financing of the implementation of the investment program entitled „Development of oncological therapy by providing innovative and comprehensive diagnostic and therapeutic methods in the Uniwersyteckim Centrum Klinicznym”, concluded on 29.04.2026 between the State Treasury – Minister of Health, and the Uniwersyteckim Centrum Klinicznym.
