Poland – Antiseptics and disinfectants – dostawa preparatów do żywienia dojelitowego i pozajelitowego oraz mleka modyfikowane wraz z osprzętem

Subject of the contract is the supply of disinfectants, the detailed requirements and quantities of which are specified in Annex 2 to this SWZ Formularz assortment-price. 1) The contract will be executed over the next 24 months. 2) The expiry date and batch number must be marked on the packaging of each disinfectant. 3) The expiry date and batch number must be indicated on the invoice or on a document attached to the invoice. 4) The ordering party will place the order by e-mail. 5) Delivery to the store of the Pharmacy at the ordering party's headquarters will be made at the contractor's expense and risk 1.1. The subject of the contract should: 1) Have a current marketing authorisation issued by the Minister of Health. 2) Be supplied in original single-unit packaging. 3) Have labelling that is clear, unambiguous, truthful, written in Polish, on the label. 4) Have labelling on the label containing: • the name of the preparation and active substances, • the expiry date, • recommendations regarding the scope of use and biocidal action, • the phrase “before use, read the enclosed information leaflet” in cases where the size of the packaging prevents the inclusion of all essential information, • the CE marking and the identification number of the notifying body, if it has issued the certificates of conformity (applies to the preparation classified as a medical device) 5) The ordering party does not agree to the offer of preparations packaged in other packaging than those indicated, except for items where the ordering party has allowed changes.
