The subject of the contract is the successive delivery of accessories: sensors, electrocardiographic leads, intermediate wires, SpO2, NIBP cuffs for cardiomonitors, and ECG electrodes/leads for medical equipment owned by the Contracting Authority. The detailed description of the subject of the contract is in the Product and Price Form, which is an annex to the SWZ. The Contractor submits an offer by completing the Product and Price Form (data should be entered in a font size of at least 10). 2. The Contracting Authority requires that the offered assortment meets the conditions specified in the relevant laws, in particular in the Act of 7 April 2022 on medical devices, and: a) has – according to the class - current certificates of a notified body or declarations of conformity – this applies to the offered medical devices, b) has documents confirming market approval, information leaflets and other documents specified by law, which the Contractor undertakes to submit to the Contracting Authority immediately upon its request - this applies to the offered medical devices.
