The subject of the contract is the successive delivery of infusion fluids for the needs of the Voivodeship Hospital Complex named after dr. Roman Ostrzycki in Konin. The subject of the contract is detailed in Annex 2 to the SWZ (Product-Cost Form), which specifies the quantity of the subject of the contract and the description of the subject of the contract. The Contracting Authority allows for the submission of partial offers. The subject of the contract has been divided into 28 parts (packages). The Contracting Authority allows for the submission of an offer for one, several, or all parts. The subject of the contract will be delivered at the cost, risk, and transport of the Contractor. The conditions for the implementation of the Subject of the Contract are also included in the draft provisions of the contract, which constitute Annex 5 to the SWZ. The minimum guaranteed level of order value, to which the Contracting Authority commits (the minimum guaranteed level of contract value utilization), has been determined in the Draft Contract Provisions constituting Annex 5 to the SWZ.
