Poland – Non-hazardous refuse and waste treatment and disposal services – Zagospodarowanie komponentów do produkcji RDF – odpadów o kodach 19 12 12 i 19 12 04 wraz z usługą ich odbioru i transportu

The subject of the contract is the service of managing components for the production of RDF – waste with codes 19 12 12 and 19 12 04 along with the service of their collection and transport in an estimated total quantity of 2 500 Mg. 2) The description of the subject of the contract, the description of the requirements of the contracting authority in terms of implementation and acceptance are specified by: - description of the subject of the contract – Annex 1 to SWZ, - the planned provisions of the contract – Annex 2 to SWZ. 3) The contracting authority does not foresee the obligation for the contractor to conduct a site visit or for the contractor to verify the documents necessary for the implementation of the contract available on site at the contracting authority. The contracting authority foresees the possibility of conducting a site visit of the place of contract performance. The place of the site visit: the area of the Municipal Waste Treatment Plant "Orli Staw", Orli Staw 2, 62-834 Ceków. Contractors interested in conducting the above site visit to familiarize themselves with the facility and waste – in connection with the implementation of the subject of the contract, are obliged to declare their willingness to conduct the visit through the procurement platform by sending a message to the contracting authority. 4) The contracting authority, in accordance with art. 139 of the Pzp, will first conduct an examination and evaluation of the offers, and then conduct a subjective qualification of the contractor whose offer was the highest rated, in terms of the absence of grounds for exclusion and the fulfillment of the conditions for participation in the procedure. 5) The offer together with the annexes must be prepared in Polish. The offer should be prepared on the Offer Form or according to the same scheme, which is Annex 3 to SWZ. The offer should be submitted, under pain of invalidity, in electronic form, i.e. in electronic form with a qualified electronic signature. 6) The contractor also attaches to the offer: a) a declaration of non-exclusion and fulfillment of the conditions for participation in the procedure (JEDZ), as mentioned in point III.3.1)1. SWZ; b) Declarations regarding the grounds for exclusion from art. 5k of Regulation 833/2014 and art. 7 of the Act on special solutions in the field of counteracting the support of aggression on Ukraine and serving to protect national security, as mentioned in point III.3.1)10. SWZ (the template is Annex 5 to SWZ); c) documents confirming the authority to represent the contractor/ contractors jointly applying for the grant of the contract, as mentioned in point I.2. SWZ – if applicable; d) Declaration of contractors jointly applying for the grant of the contract, as mentioned in point I.1.4)b) SWZ (the template is Annex 7 to SWZ) – if applicable e) Proof of submission of objections, as mentioned in III.4. SWZ. f) Business secrecy reservation – if applicable – in a situation where the offer or other documents submitted during the procedure will contain business secrets, the contractor, together with the transmission of such information, reserves that they may not be made available, and demonstrates that the reserved information constitutes a business secret within the meaning of the provisions of the Act of 16 April 1993 on combating unfair competition. 7) Changes to the contract are included in §11 of the planned provisions of the contract – Annex 2 to SWZ. 8) Before concluding the contract, the contractor, among other things, will present current and paid civil liability insurance, in the scope of the conducted activity related to the subject of the contract, in the amount of not less than 1 700 000,00 zł.
