The subject of the public procurement contract includes the selection of a contractor for a single delivery of patient immobilization devices compatible with existing equipment, namely: 6D couch on Varian TrueBeam EDGE accelerator, patient table on Varian Clinac IX accelerator, Siemens Somatom Definition AS CT scanner and Accuray Tomotherapy HDA helical tomotherapy unit, as follows:

1. A fibreglass base plate allowing for the attachment of base plates and masks for head and shoulder immobilization, for use with "push-pin" type thermoplastics masks. The base plate should be compatible with the 6D couch on Varian TrueBeam EDGE accelerator, patient table on Varian Clinac IX accelerator, Siemens Somatom Definition AS CT scanner and Accuray Tomotherapy HDA helical tomotherapy unit. Through a fixing bar (locating bar) with a rectangular slot for head base plates - quantity - 3

2. A full set of head base plates with different curvatures for immobilizing the head in the "prone" position and the "supine" position, compatible with the base plate for "push-pin" type masks. The base plates should be made of foam and have a rectangular base for attachment - set - 3

3. A set of blocks and wedges for changing the height and tilt of the base plates from point 2, with a rectangular base for attachment to the base plate for the head and neck - set - 3

4. A wedge for positioning during breast irradiation with arm supports and the possibility of changing the tilt. The wedge should be made of foam - quantity - 3

4.1 Wedges at 5° and 10°, which allow for combination to raise by 15°.

4.2 A system for positioning the hands above the patient's head including handles for gripping allowing movement to different positions

5. Immobilization system for irradiating volumes in the thoracic region (so-called "wing board") with the possibility of positioning the hands above the patient's head with handles for gripping allowing movement to different positions - quantity - 3

6. A system for positioning the knees and ankles, made of foam, containing: - quantity - 3

7. Bolus from gelled gel

7.1 30x30x5 mm without packaging - quantity - 5

8. Thermostat-controlled tank for thermoplastics masks; minimum internal dimensions of the working tank (length x width) - 660mm x 550mm - quantity - 1

9.2 Microporous thermoplastics immobilization mask for the head, neck and shoulders with 5-point attachment for single use, compatible with the "push-pin" type base plate - quantity - 250

9.3 Microporous thermoplastics mask for the head, neck and shoulders reinforced for radiotherapy with an "open face" type 3-point attachment, for single use, compatible with the "push-pin" type base plate - quantity - 50

9.4 Microporous thermoplastics mask for the head, neck and shoulders reinforced for radiotherapy with an "open face" type 5-point attachment, for single use, compatible with the "push-pin" type base plate - quantity - 30

9.5 Thermoplastics immobilization mask for the abdomen and pelvis with 4-point attachment, for single use, compatible with the available system in the clinic - quantity - 50

9.6 Microporous thermoplastics mask for the head, neck and shoulders reinforced for radiotherapy, front and back, for single use, compatible with the available Encompass base plate in the clinic - quantity - 50

10. Fixing bar (so-called "locating bar") with the possibility of moving 10 cm left-right suitable for Varian TrueBeam EDGE accelerator, Varian Clinac IX accelerator, Siemens Somatom Definition AS CT scanner and Accuray Tomotherapy HDA helical tomotherapy unit - quantity - 4

To verify the compliance of the offered items with the requirements of the Contracting Authority, each participant must submit samples of the offered items, by positions from 9.1 to 9.6 from the Table in section SUBJECT OF THE CONTRACT of the present documentation (one quantity of the items by the indicated positions, with the provided samples not being packaged in factory packaging). No variations of the samples are allowed. The procedure is carried out in accordance with the order of Article 104, paragraph 2 of the Public Procurement Act (so-called "reverse order" of consideration - the evaluation of the technical and price proposals of the participants is carried out before the preliminary selection).
