Full title: Conducting supervision of the social support network for educators of Children's Homes within the training on medical issues based on EBM for participants in the project entitled: „Na ratunek – kadry DIM” co-financed by the European Union, from the European Social Fund Plus within the European Social Development Funds programme

1. The subject of the present contract is the conducting of supervision of the social support network for educators of Children's Homes within the training on medical issues based on Evidence Based Medicine (EBM), for participants in the project „Na ratunek - kadry DIM", co-financed by the European Union from the European Social Fund Plus within the European Social Development Funds programme, planned for implementation.

2. The present contract (Part 1) is part of a contract awarded in parts, each of which is the subject of a separate procedure. The detailed description of the subject of the contract in relation to the added part of the contract is contained in Annex 1A-1B and Annex 4 to the SWZ.

3. The contractor is obliged to implement the contract on the principles and conditions described in the SWZ and in Annex 1A-1B to the SWZ and the contract template, constituting Annex 4 to the SWZ.

4. Note: due to the organisational assumptions of the training indicated in the detailed schedule of implementation of the EBM-based training, constituting an annex to the Description of the subject of the contract, the contractor ensures the availability of a person designated to implement the given part of the contract, meeting the minimum substantive requirements specified by the Contracting Authority in Section VIII, point 1, paragraph 4 of the SWZ and in Annex 1B to the SWZ in the scope of the given part of the contract and the implementation of supervision, for the indicated groups of participants, in the manner and within the deadlines indicated in the detailed schedule of implementation of the training, constituting an annex to the Description of the subject of the contract.

6. Minimum guaranteed level of contract implementation: The Contracting Authority guarantees the implementation of the contract at the level of: implementation of 1 hour of supervision for the given part of the contract.
