Poland – Antineoplastic agents – Zakup i sukcesywna dostawa leków onkologicznych oraz leków różnych.

The subject of the contract is the purchase and successive delivery of oncological drugs and various other drugs, specified in the parts, for the University Clinical Hospital named after Fryderyk Chopin in Rzeszow.

The detailed description of the subject of the contract, the requirements of the Purchaser are specified in the Price- Offer Forms - Annexes no. 1 to the SWZ for individual parts, which are an integral part of the SWZ.

The Purchaser requires the subject of the contract to be offered, which:

3.1. meet the requirements described in the Price- Offer Forms,

3.2. meet the requirements described in the SWZ,

3.3. in the case of medicinal products covered by refund - are covered by the Act of 12 May 2011 on the refund of medicines, special dietary foods and medical devices.

The scope of the contract covers the delivery of the subject of the contract in the quantities specified in the Price- Offer Forms for the period:

- for Part from no. 1 to no. 21 – 12 months from the date of signing the contract.

5. CONTRACT PERFORMANCE DEADLINE:

5.1 The subject of the contract in the scope of Part from no. 1 to no. 21 will be implemented in the form of successive deliveries for a period of 12 months from the date of signing the contract.

5.2 In the event that during the validity of the contract the subject of the contract is not fully implemented in relation to Part from no. 1 to no. 21, the Purchaser reserves the right to extend the period of validity of the contract. The change in the validity period of the contract will take place by means of a written annex signed by the Parties. The Purchaser will inform the Contractor of the intention to extend the period of validity of the contract before the expiry of the deadline for which it was concluded.

6. Deadline for submitting a request for clarification of the content of the SWZ: 15.10.2026.
