1. The subject of the contract is the supply of medical devices for Interventional Radiology for UCK in the assortment, estimated quantity and requirements specified in Annex 3 to the SWZ. 2. The contracting authority allows the submission of partial offers. The term "partial offer" refers to individual packages established in Annex 3 to the SWZ. The contractor may submit an offer for one, several or all packages. 3. The contracting authority requires that the offered assortment meets the requirements regarding medical devices contained in the Act of 7 April 2022 on medical devices (Journal of Laws of 2024, item 1620) and in the executive acts issued on the basis of this Act. 4. The offered assortment must meet the requirements specified in Annex 3 to the SWZ. Failure to meet even one of the requirements specified in Annex 3 to the SWZ will result in the rejection of the offer. 5. The delivery of the assortment of this contract will be carried out on the basis of partial orders. The maximum delivery time is 6 working days from the date of the order placed by the contracting authority. 6. The assortment referred to in paragraph 1, the contractor will be obliged to deliver at their own expense to the Medical Supply Department, ul. Smoluchowskiego 17.
