Poland – Analysers – Dzierżawa analizatorów wraz z dostawą odczynników oraz dostawa testów do Laboratorium Diagnostycznego szpitala Olmedica w Olecku Sp. z o. o.

1. The subject of the contract is: "Rental of analyzers along with the supply of reagents and supply of tests to the Diagnostic Laboratory of Olmedica Hospital in Olecko Sp. z o. o." 2. The detailed description of the subject of the contract is specified in the description of the subject of the contract constituting Annex No. 4 to the SWZ and in the contract template constituting Annex No. 2, 2a and 2b to the SWZ. 3. The estimated quantities specified in Annex No. 4 to the SWZ may be reduced, depending on the requirements of the Buyer. The Buyer is not obliged to order the remaining part of the goods not realized during the contract period due to reduced demand. 4. The subject of the contract must meet the requirements specified in the Act of 07 April 2022 on medical devices (i.e. Journal of Laws 2024 item 1620 as amended), must have documents allowing for trade and use in Poland, in particular, must be marked with the CE mark, and if the conformity assessment was carried out under the supervision of a notified body, the identification number of the relevant body must be placed next to the CE mark. The Contractor, upon each request of the Buyer, will present certified copies of the permits (certificates) in accordance with the original, and if the subject of the contract has not been classified as a medical device and, in accordance with European directives and the Act on medical devices, is not subject to declarations of conformity and is not subject to any entry in the register, will submit the appropriate declarations to the Buyer when the Buyer so requires. 5. The Buyer requires that the offered products meet the requirements specified by the applicable law, have been admitted to trade, and possess the required valid documents, certifying admission for use in Poland.
