Description
The subject of the contract is the supply of two brand-new rescue ambulances with a DMC of over 3.5 tonnes, complete with equipment. 2. The medical equipment includes the supply of each ambulance equipped with: a) Electric-hydraulic transport stretcher, b) Non-powered transport chair, c) Transport ventilator, d) Infusion pump, e) USG head, f) Defibrillator, one of each per ambulance. 3. A detailed description of the subject of the contract is contained in Annex No. 2 to SWZ – Detailed description of the subject of the contract – Technical form. The requirements specified in this annex apply to each of the two ambulances that are the subject of the contract. The contractor is obligated to deliver two complete, brand-new ambulances with medical equipment, with each ambulance being equipped with all the devices listed in point 2, one of each type. The total delivery includes two ambulances and two of each of the six types of medical equipment. 4. When specifying requirements in terms of standards, the purchaser indicates this as a requirement to comply with these standards (PN-EN 1789+A1:2024-06) or equivalent – in the scope of each part of the description of the subject of the contract, or assortments, in which such standards have been specified. 5. The contractor is obligated to ensure and perform warranty inspections of the medical installation of each of the two ambulances throughout the warranty period, with a frequency of no less than once a year, and if the manufacturer of the installation requires inspections to be performed more frequently – in accordance with the manufacturer's requirements. All costs associated with performing inspections, including labor costs, operational materials required to perform the inspection, service travel, and other actions necessary to maintain the warranty, are borne by the contractor. These costs must be included in the offer price and cannot be the basis for charging additional fees to the purchaser. 6. The contractor is obligated to ensure and perform, at their own cost, throughout the warranty period granted for each medical device constituting the equipment of the ambulances, periodic technical inspections and service actions required by the manufacturer of the given device. 1) The obligation mentioned above applies to each of the medical devices delivered as part of the contract, i.e., electric-hydraulic transport stretchers, transport chairs, transport ventilators, infusion pumps, USG heads, and defibrillators, totaling 12 devices. 2) Technical inspections and service actions must be performed with a frequency specified by the manufacturer of the given device, but no less than once every 12-month period of the warranty. If the manufacturer requires inspections to be performed more frequently than once a year, the contractor is obligated to perform them in accordance with the frequency specified by the manufacturer. 3) Inspections and service actions must be performed by an entity possessing the qualifications and authorizations required by the manufacturer of the given device, in accordance with the technical documentation, user instructions, and applicable legal regulations. 4) The scope of inspections includes all actions required by the manufacturer of the given device, necessary to ensure its correct and safe use and to maintain warranty entitlements, including, if required, technical controls, safety tests, calibrations, adjustments, software updates, and replacement of parts and operational materials intended for replacement as part of periodic inspections. 5) All costs associated with performing mandatory inspections and service actions, particularly labor costs, service travel, device transport, required tests, calibrations, adjustments, software updates, and parts and materials subject to replacement as part of periodic inspections, are borne by the contractor. 6) The contractor is obligated to include the full costs of performing the obligations specified in this clause in the offer price. The purchaser will not incur any additional costs for performing periodic inspections and service actions during the warranty period. 7) The contractor is obligated to coordinate the inspection dates with the purchaser with sufficient advance notice, in a manner that ensures the performance of actions within the times required by the manufacturer and minimizes to the necessary extent the removal of equipment from use. 8) For each inspection performed, the contractor is obligated to prepare and submit to the purchaser a service protocol or other document confirming the performance of the required actions, containing at least the identification of the device, the date of the inspection, the scope of the actions performed, the results of the control, and the date of the next required inspection. 9) The obligation to ensure inspections and service actions covers the entire warranty period granted for the individual medical devices, even in the case of offering a warranty period longer than the required minimum of 24 months.
