Poland – Disposable non-chemical medical consumables and haematological consumables – PN 98/26 - Dostawa sprzętu jednorazowego użytku

1. The subject of the contract is the supply of single-use equipment I (including „komis” – applies to task no. 65, task no. 66). 2. The subject of the contract has been divided into 66 tasks – in accordance with Annex No. 1 to SWZ „Assortment-price Form”. 3. A detailed description of the subject of the contract and the required parameters, required size, feature, unit, quantity and quantity planned for purchase under the option right is specified in the Assortment-price Form, which is Annex No. 1 to SWZ. The parameters described in Annex No. 1 to SWZ, as required parameters, constitute the boundary requirements, the non-compliance of which will result in the rejection of the offer. 4. Detailed conditions and rules for the implementation of the subject of the contract are specified in the Contract Template - Annex No. 5 to SWZ. 5. The contractor will deliver the subject of the contract to the Pharmacy Store of the Hospital. 6. Contract performance period: 18 months from the date of signing the contract. 7. The offered single-use equipment is a medical device and must be approved for sale and use in accordance with the conditions specified in the Act of 07.04.2022 on medical devices (i.e.: Dz.U. of 2024 item 1620), implementing acts thereto and must have a Declaration of Conformity. For task no. 37, task no. 46 and task no. 63 item 7 and 14: a) when the offered subject of the contract is a medical device it must be approved for sale and use in accordance with the conditions specified in the Act of 07.04.2022 on medical devices and implementing acts thereto and must have a Declaration of Conformity. b) when the offered subject of the contract is a cosmetic product it must have an entry in the Cosmetic Products Notification Portal (CPNP) Register (required name of the offered product and number assigned upon registration). 8. Delivery deadline – according to current needs, each time within a maximum of 2 working days from the date of the order.
