4.1 The subject of the contract is the successive supply of consumables for printers, copiers, multifunctional devices in 2027. 4.2 A detailed description of the subject of the contract is provided in Annex No. 1 to SWZ and Annex No. 5 to SWZ. 6. CONTRACT PERFORMANCE TERM 6.1 Contract performance term: from the date of signing but not earlier than 04.01.2027 to 31.12.2027 or until the contract amount is exhausted, whichever occurs first. 7. GROUNDS FOR EXCLUSION FROM THE PUBLIC PROCUREMENT PROCEDURE are described in section 7 of SWZ. 8. INFORMATION ON THE CONDITIONS FOR PARTICIPATION IN THE PROCEDURE FOR AWARDING THE CONTRACT 8.1 Contractors who meet the following participation conditions may apply for the contract: 8.1.1 ability to operate in the economic market: The Contracting Authority does not specify any requirements in this regard that the Contractor is obliged to demonstrate in a special manner. 8.1.2 authorisation to carry out a specific economic or professional activity, if this results from separate provisions: The Contracting Authority does not specify any requirements in this regard that the Contractor is obliged to demonstrate in a special manner. 8.1.3 economic or financial situation: The Contracting Authority does not specify any requirements in this regard that the Contractor is obliged to demonstrate in a special manner. 8.1.4 technical or professional capacity: The Contracting Authority does not specify any requirements in this regard that the Contractor is obliged to demonstrate in a special manner. 8.2 The Contracting Authority will first carry out an examination and evaluation of the tenders, and then qualify the Contractor whose tender has been most highly evaluated, in terms of the absence of grounds for exclusion and the fulfilment of the conditions for participation in the procedure, based on Article 139(1) of the Pzp Act. 9.1 List of declarations and documents that the Contractor submits together with the tender: 9.2 The Contractor is obliged to submit, together with the tender, in order to preliminarily confirm that it is not subject to exclusion and meets the conditions for participation in the procedure: 9.2.1 Declaration of the Contractor regarding the grounds for exclusion from Article 5k of Regulation 833/2014 as amended by Regulation 2022/576 and Article 7(1) of the Act on special solutions in the field of counteracting the support of aggression against Ukraine and serving to protect national security, submitted pursuant to Article 125(1) of the Pzp Act, in accordance with the template constituting Annex No. 7 to SWZ. In the case of contractors jointly applying for the contract, this declaration is submitted by each of these contractors (this also applies to partners of a civil partnership). 9.9 Information on certification is provided in section 9.9 of SWZ. 15. TENDER SUBMISSION METHOD The tender consists of the following documents: 15.1 Tender Form – prepared according to the template - Annex No. 2 to SWZ (signed with a qualified electronic signature). 15.2 Price Form calculation sheet file Annex No. 2a to SWZ - (signed with a qualified electronic signature). Note: 1. Failure to attach Annex No. 2a to SWZ price form, failure to complete all its positions necessary for calculating the price or failure to sign the aforementioned annex with a qualified electronic signature - will result in the rejection of the tender by the Contracting Authority. 2. It is required to complete all the indicated positions of the Price Form necessary for calculating the price. 15.3 Declaration of the Contractor regarding the grounds for exclusion from Article 5k of Regulation 833/2014 as amended by Regulation 2022/576 and Article 7(1) of the Act on special solutions in the field of counteracting the support of aggression against Ukraine and serving to protect national security, submitted pursuant to Article 125(1) of the Pzp Act, in accordance with the template constituting Annex No. 7 to SWZ (signed with a qualified electronic signature).
