The subject of the Contract is the ongoing supply of special medical material in accordance with the provisions of Section 2 of Act No. 375/2022 Coll., on medical devices and in vitro diagnostic medical devices (hereinafter referred to as "ZZP"). Specifically, the subject of the Contract is the ongoing supply of sets of short-term mechanical circulatory support, including the required accessories (hereinafter referred to as "Medical Material", "Medical Devices" or "ZP") for Krajská nemocnice Liberec, a.s., Department of Interventional Cardiology, in accordance with the TD for a period of 2 years. The contract also includes the free loan of 2 control units of the short-term mechanical circulatory support system, including related accessories, for a period of 24 months. The Loan Agreement Template is Appendix No. 6 of the TD.
