The subject of the contract is: Delivery of single-use and multiple-use medical articles for SPZZOZ in Nowa Dęba. Type of contract – SUPPLY. A detailed description and scope of the contract subject is contained in Annex 2 to SWZ – Description of the Contract Subject - Product and Price List - Packages 1 – 42. Package 1: Hernia mesh, Package 2: Sterile single-use medical articles, Package 3: Sterile single-use medical articles, Package 4: Medical articles, Package 5: Non-sterile medical articles, Package 6: Medical articles, Package 7: Sterile single-use medical articles, Package 8: Sterile single-use medical articles – intubation tubes, masks, breathing circuits, filters, etc., Package 9: Sterile single-use medical articles – blades, scalpels, diuresis measurement system, etc., Package 10: Sterile single-use medical articles – devices, syringes, etc., Package 11: Sterile single-use medical articles – intravenous infusion, Package 12: Single-use medical articles – mounts, Package 13: Containers for hospital waste, Package 14: Diapers, Package 15: Papers for CTG, ECG, etc. devices, Package 16: Various medical articles, Package 17: Gels and electrodes for examinations, Package 18: Single-use and multiple-use medical articles, Package 19: Medical thermometers, Package 20: Medical articles for the Computer Tomography Laboratory, Package 21: Laboratory articles for the closed system, Package 22: Various laboratory articles, Package 23: Protective assortment – masks, overalls, protectors, etc., Package 24: Medical articles – Operating Theatre, Package 25: Medical assortment for electro-surgery, Package 26: Sterile single-use medical articles - needles, Package 27: Sets of drapes, towels and other medical articles for the needs of the Operating Theatre, Package 28: Single-use medical gloves, Package 29: Medical articles - Endoscopy, Package 30: Medical articles, Package 31: Medical articles - OAiIT, Package 32: Medical articles, Package 33: Sterile single-use medical articles, Package 34: Medical articles, Package 35: Sterile single-use medical articles – syringes, Package 36: Articles for renal replacement therapy, compatible with the PRISMAFLEX device, for OAiIT, Package 37: Syringes for infusion pumps, Package 38: Medical sets for the needs of the Operating Theatre, Package 39: Medical articles for the needs of the Operating Theatre, Package 40: Set for precise control of gravitational flow, Package 41: Hernia mesh for procedures performed by laparoscopic method, Package 42: Medical articles OAiIT. The quantities indicated in the Description of the Contract Subject - Product and Price List Annex 2 to SWZ – are indicative quantities, adopted for the purpose of comparing offers and selecting the most advantageous offer. The Purchaser reserves the right to purchase a smaller quantity of the contract subject than indicated in Annex 2, due to the inability to predict the full demand for the contract subject covered by this SWZ. The remaining conditions regarding the implementation of the contract have been specified in the Contract Template – Annex 4 to SWZ and other annexes to SWZ. Information on equivalent solutions: a) Trade name – if a trade name of a product/firm/item appears in any annex to SWZ, it means that the Purchaser was unable to sufficiently accurately describe the subject of the contract using sufficiently accurate data and serves only to determine the desired standard of delivery; b) In the event that the Purchaser specifies the subject of the contract by indicating trademarks, patents or origin, source or a special process that characterizes the products or services provided by a specific Contractor, if it could lead to the favouring or elimination of some Contractors or products - the possibility of submitting equivalent offers is allowed. c) In situations where the Purchaser describes the subject of the contract by referring to standards, European technical approvals, certificates, technical specifications and technical reference systems, as mentioned in art. 101.1.2 and 3 of the Pzp Act, equivalent solutions to those described are allowed. d) The above definition of the subject of the contract is only auxiliary in the preparation of the offer and is intended to indicate the expected standards of the minimum technical parameters of the expected materials and devices. e) By an equivalent offer, one should understand an offer with technical, strength, quality, and performance parameters not worse than the description indicated by the Purchaser in the description of the subject of the contract. f) The parameters indicated by the Purchaser are minimum, boundary parameters. The term "parameters" refers to functionality, purpose, colour, structure, materials, shape, size, safety, durability, etc. g) In connection with the above, the Purchaser allows the possibility of offering materials with other trademarks, patents or origin, but not with other properties and functionalities than those specified in the SWZ. h) Whenever a standard is indicated in this SWZ or annexes to the SWZ, it should be assumed that the formulation "or equivalent" has been used in relation to it. i) In the event that the Contractor does not submit documents on the use of other materials and devices in the offer, it is understood that the materials and devices proposed in the description of the subject of the contract have been included in the offer price calculation and for the performance of the contract. The Purchaser allows the submission of partial offers for all parts of the contract with a division into individual Packages from 1 to 42 (none of the Packages is subject to division).
