The subject of the contract is the comprehensive equipment of Hospital Department No. VII, i.e.: measurement, design, delivery with installation of furniture and other elements constituting the entire equipment, divided into 8 tasks: Task 1 – Upholstered furniture and seating Task 2 – Office furniture and custom-made furniture Task 3 – Social installations, kitchen furniture and white goods Task 4 – Auxiliary and sanitary equipment Task 5 – Shelves and stainless steel equipment Task 6 – Medical steel furniture and medical installations Task 7 – RTV equipment Task 8 – remaining assortment - The detailed description of the subject of the contract for Task 1-8 includes: 1) Annex No. 1 to SWZ – Price list with a description of the subject of the contract (OPZ) 2) Annex No. 1A to SWZ – Item list with a description of the subject of the contract – Division into rooms. 3) Annex No. 1B to SWZ – Detailed OPZ. The detailed conditions and rules for the implementation of the subject of the contract are specified in the provisions of the Contract Template - Annex No. 5 to SWZ Contract Template. The contract execution deadline for Tasks 1-8: by 31 August 2026. Important information!!! Local view - concerns Task 2, 3, 6 1. The Contracting Authority, during the tender procedure, in accordance with art. 281.2.12 of the PZP Act, provides for the mandatory requirement to conduct a local view of the delivery and installation site and the necessity to carry out measurements of the subject of the contract, in order to obtain all data that may be useful for preparing an offer and implementing the subject of the contract. The necessity of the requirement to conduct a local view results from the specificity of the subject of the contract, which is: comprehensive equipment of Hospital Department No. VII, i.e.: measurement, design, delivery with installation of furniture and other elements constituting the entire equipment. The purpose of the mandatory local view is to enable Contractors to familiarize themselves with the conditions for the implementation of the contract. Conducting the view is a mandatory condition for submitting a correct offer. 2. An offer submitted by a Contractor without conducting a local view will be rejected on the basis of art. 226.1.18 of the PZP Act. 3. The Contracting Authority sets the following dates for the local view: 01.07.2026; 08.07.2026; 15.07.2026; 20.07.2026. Note! additional view for task no. 2: 21.07.2026 4. A Protocol will be drawn up from the local view, signed by the parties, which will confirm the local view for Tasks 2, 3, 6 (the Protocol template from the local view is Annex No. 8 to SWZ).
