The subject of the contract is the delivery of 1,026.5 tonnes of filtration sands and gravels (hereinafter also referred to as the "Product") - used for the treatment of water intended for human consumption, by the Contractor's transport. 2. The Detailed Description of the Contract Subject (hereinafter also referred to as the DCS) is contained in Annex No. 5 to the Contract Notice. 3. The detailed conditions for the implementation of the Contract are contained in Annex No. 6 to the Contract Notice. 4. Subject evidence: 4.1. In order to prove that the offered Product complies with the requirements specified in Annex No. 5 to the Contract Notice – Description of the Contract Subject, the Contracting Authority requires the Contractor to submit, together with the offer, the following subject evidence: 1) Current hygiene certificate issued by the National Institute of Public Health – National Institute of Hygiene, allowing the Product to be used in water intended for human consumption. Note: The Contracting Authority will accept equivalent subject evidence if it confirms that the offered supplies meet the requirements specified in the DCS. 4.2. The Contracting Authority reserves the right, in the event that the Contractor does not submit the subject evidence referred to above, or if the submitted subject evidence is incomplete, to invite the Contractor, under Article 107(2) of the Act, to submit or complete it within the specified deadline.
