The subject of the contract is the delivery of 6 medical ambulances with 4x4 drive, meeting the requirements of standard PN-EN 1789+A2 or an equivalent standard, along with medical equipment meeting the requirements of standard PN-EN 1865 or an equivalent standard. The purchase of ambulances along with medical equipment (Packages 1–5) will be co-financed with a specific grant awarded by the Podkarpackie Voivodeship through funds planned in the 2026 budget of the Podkarpackie Voivodeship.

EVIDENTIAL MEANS - Evidential means are understood to be means serving to confirm the conformity of the offered supplies with the requirements, characteristics or criteria specified in the description of the subject of the contract or the description of the criteria for evaluating offers, or requirements related to the implementation of the contract. a) The Contracting Authority requires the submission of other evidential means, apart from the label and certificate, pursuant to Article 106 of the Pzp Act. The Contractor applying for the award of the contract is obliged to submit, together with the offer, the following evidential means confirming that the offered supply meets the requirements specified by the Contracting Authority, i.e. a declaration confirming that the offered vehicle meets the requirements specified by the Contracting Authority in the description of the subject of the contract, i.e. detailed qualitative, technical and operational data indicating the parameters of the offered vehicle required by the Contracting Authority, in accordance with - Annex 2A to SWZ (a list of technical parameters constituting at the same time a detailed description of the subject of the contract) for Package 1 - Annex 2B to SWZ (a list of technical parameters constituting at the same time a detailed description of the subject of the contract) for Package 2 - Annex 2C to SWZ (a list of technical parameters constituting at the same time a detailed description of the subject of the contract) for Package 3 - Annex 2D to SWZ (a list of technical parameters constituting at the same time a detailed description of the subject of the contract) for Package 4 - Annex 2E to SWZ (a list of technical parameters constituting at the same time a detailed description of the subject of the contract) for Package 5, - Annex 2F to SWZ (a list of technical parameters constituting at the same time a detailed description of the subject of the contract) for Package 6. Evidential means should be submitted in electronic form with a qualified electronic signature. The Contracting Authority does not provide for the possibility of supplementing the evidential means, in accordance with Article 107(3) of the Pzp Act, due to the fact that they serve to confirm the conformity of the offered supplies with the requirements specified in the description of the subject of the contract (OPZ). Evidential means serve to confirm the conformity of the offered supplies with the requirements specified by the Contracting Authority and will be assessed on the basis of their content on the date of submission of offers. In the event that the description of the subject of the contract contains references to standards, technical assessments, technical specifications and technical reference systems, as mentioned in Article 101(1)(2) and (3) of the Act, the Contracting Authority allows equivalent solutions to those described. The Contracting Authority requires the subject of the contract to be executed within the following deadlines, counted from the date of conclusion of the contract: Package 1 – 20 calendar days, Package 2 – 20 calendar days, Package 3 – 60 calendar days, Package 4 – 60 calendar days. Package 5 - 60 calendar days. Package 6 - 60 calendar days. As an additional criterion for evaluating offers, the Contractor may declare a reduction in the deadline for implementing the subject of the contract. A declaration in this respect should be submitted in the tender form constituting respectively: Annex 1A – for Package 1, Annex 1B – for Package 2, Annex 1C – for Package 3, Annex 1D – for Package 4, Annex 1E – for Package 5, Annex 1F – for Package 6, to the Specification of Contract Terms (SWZ). The shorter implementation deadline declared by the Contractor will be taken into account in the evaluation of offers within the accepted criterion "delivery time". The Contracting Authority requires that the selected Contractor conclude the contract on the terms specified in the draft contract PPU, constituting: Annex 9 to SWZ common to all parts (packages). In the event of concluding a contract for Package 6, the provisions regarding co-financing from the budget of the Podkarpackie Voivodeship for 2026 and the logo will not apply and will be removed. The type and scope of changes to the contract and the conditions for their introduction are contained in Annex 9 - § 10. The period of binding of the offer is 90 days - i.e. until 17.11.2026.
