The subject of the contract is Medical services including preventive medical examinations (initial, periodic, control, sanitary-epidemiological, diagnostic tests, laboratory tests, psychotechnical tests, specialist consultations) for professional soldiers and employees of the Ministry of National Defence, 43 Military Economic Department, and professional soldiers and employees of JW and institutions under the supply of 43WOG. 2. The contracting authority informs that it envisages the use of optional orders (right of option), in accordance with the provisions of the contract template constituting Annex No. 8 to the SWZ. The contracting authority in Annex No. 6a-6c to the SWZ (price forms) specifies the basic level of the order, which it may extend by increasing the quantity for: Part 1 to 150% of the basic order value. Part 2 to 150% of the basic order value. Part 3 to 100% of the basic order value. 3. A detailed description of the subject of the contract is in Annex No. 9 to the SWZ.
