The subject of the contract is the provision of regular care services at the residence of the beneficiaries of the Social Services Centre in Gorzów Wielkopolski, within the city of Gorzów Wielkopolski. The services will be provided to persons who, due to age, illness or other reasons, require the help of others, pursuant to art. 50 of the Act of 12 March 2004 on social assistance and individual administrative decisions. The contract does not include specialist care services, specialist care services for persons with mental disorders or neighbourly services. The scope of the services includes, in particular, assistance in meeting daily living needs, hygienic care, nursing care recommended by a doctor not requiring qualifications specific to medical professions, and, where possible, ensuring contact with the environment. The detailed scope, standards, organisation and documentation of the services are specified by the SWZ, including Annex No. 1 to the SWZ - Detailed description of the subject of the contract. Contract size: The estimated number of beneficiaries is approximately 350 persons per month/ The estimated number of service hours is approximately 7,000 hours per month. The planned scope of the contract in the basic implementation period of 24 months is 168,000 hours, including the minimum guaranteed scope of 109,200 hours. The contracting authority envisages the possibility of renewing the contract for a total period not exceeding 24 months. The renewal may take place once for a period of 24 months or twice, each time for a period of 12 months. One hour of service means 60 minutes of actual service provision; travel time to or from the beneficiary is not included in the service time. The numbers given in the SWZ are estimated. The number of people, the size and distribution of hours and the places of provision will depend on administrative decisions and the current needs of the beneficiaries.
