ZAS seeks an integrated solution for the realization of a Care Pathway platform that can be used transmurally for the development of “patient journeys”. Such a platform includes:

• Integrations with the ZAS EPD (HiX)

• Integrations with other ZAS internal sources for the disclosure of e.g. invoicing data,

• Integrations with current and future eHealth initiatives,

• Support for 3rd party plug-ins and relevant platforms such as the Digital Referral Prescription (DRP) platform of the government to support telemonitoring or equivalent initiatives,

• Support for transmural care where the platform is also used by and for transmural care partners to support the shared patient.

• Future-proofing for in-depth digital service provision and support for transmural care.

The contract will be awarded via a competitive procedure with negotiation.
