The subject of the service is the provision, implementation and operation of a cloud-based Software-as-a-Service (SaaS) solution to support a comprehensive digital discharge and transition management. The aim is to coordinate the entire discharge management process – from early discharge planning, through case-specific coordination of follow-up care, to the creation of the discharge letter – across sectors, to avoid gaps in care, and to reduce administrative burdens in the hospital. The discharge management system must therefore meet the requirements of the Krankenhauszukunftsgesetz and ensure legally compliant documentation as well as interoperability with other systems.
