The subject of the Main Order are insurance services for the benefit of Entities, for a period of 36 months calculated separately for each mechanical vehicle reported in the I Settlement Period within the framework of the public procurement contract, referred to in § 3, paragraph 1, point 1.1 of the Contract Template constituting Annex No. 5 to the SWZ, in the following scope: 1.1. compulsory civil liability insurance for owners of mechanical vehicles, referred to in the Act of 22 May 2003 on compulsory insurances, the Insurance Guarantee Fund and the Polish Bureau of Transport Insurers (i.e. Journal of Laws of 2025, item 367 with later amendments) and in Section II, Chapter 1 of Annex No. 1 to the Contract "Description of the Subject of the Order" constituting Annex No. 2 to the SWZ, hereinafter also referred to as "Motor Third Party Liability Insurance" or "MTPL"; 1.2. Auto Casco insurance, referred to in Section II, Chapter 2 of Annex No. 1 to the Contract "Description of the Subject of the Order" constituting Annex No. 2 to the SWZ, hereinafter also referred to as "Auto Casco Insurance" or "AC"; 1.3. Driver and Passenger Accident Insurance, referred to in Section II, Chapter 3 of Annex No. 1 to the Contract "Description of the Subject of the Order" constituting Annex No. 2 to the SWZ, hereinafter also referred to as "PAI" or "PAI"; 1.4. Assistance insurance, referred to in Section II, Chapter 4 of Annex No. 1 to the Contract "Description of the Subject of the Order" constituting Annex No. 2 to the SWZ, hereinafter also referred to as "Assistance Insurance" or "ASS"; 1.5. civil liability insurance for owners of mechanical vehicles in foreign traffic, referred to in Section II, Chapter 5 of Annex No. 1 to the Contract "Description of the Subject of the Order" constituting Annex No. 2 to the SWZ, hereinafter also referred to as "Green Card" or "GC". 2. The insurance periods for mechanical vehicles have been indicated in the "Vehicle List" constituting Annex No. 9 to the SWZ "Vehicle List - cost calculation". 3. The detailed description of the subject of the Order has been specified in the Contract Template "General Contract of Transport Insurances" constituting Annex No. 5 to the SWZ and in Annex No. 1 to the Contract Template "Description of the Subject of the Order" constituting Annex No. 2 to the SWZ; 4. The Orderer, acting pursuant to Article 95, paragraph 1 of the Pzp Act, requires the Contractor or Subcontractor to employ, on the basis of an employment relationship, all persons who will be responsible for the handling of the Contract and will perform activities involving: maintaining current contact with the Insurer, the Insured or the broker (conducting correspondence within the framework of the Contract and telephone contacts regarding matters related to the Contract), issuing insurance documents, settling the Contract, confirming the scope of the Insurer's liability, confirming insurance cover, informing about the status of insurance premium settlements, providing information or explanations related to the implementation of the Contract, on the principles and to the extent specified in the Contract Template constituting Annex No. 5 to the SWZ. 5. The meaning of the terms used in the SWZ is identical with the definitions adopted in the Contract Template constituting Annex No. 5 to the SWZ and in Annex No. 1 to the Contract "Description of the Subject of the Order" constituting Annex No. 2 to the SWZ. 6. The List of Entities, for the benefit of which the procedure is conducted, includes Annex No. 8 to the SWZ. 7. Detailed data on the Entities and on the vehicles necessary for the assessment of insurance risk, including data on the subject of insurance and claims experience, have been included in the Annexes to the SWZ from No. 8 to No. 10.
